Marie Curie, in partnership with University Hospitals Plymouth, is introducing a new service model aimed at enhancing end-of-life care, with a special focus on identifying patients who are approaching the end of their lives.

This innovative service focuses on four key areas:

·       Delivering hands-on care and emotional support to patients and their families in the Emergency Department

·       Proactively identifying and assisting those nearing the end of life within the acute hospital, ensuring timely transfers to their preferred place of care

·       Offering specialised care within Mount Gould Community Hospital

·       Engaging a dedicated team of volunteers to provide companionship and advance care planning.

This is an exciting time to be joining us as a Registered Nurse, where you will play a crucial role in achieving these outcomes and making a meaningful difference in the lives of patients and their families.

In this role, you will work closely with teams and patients’ loved ones to provide comprehensive, person-centred care for those nearing the end of life. This includes conducting holistic assessments, developing advance care and treatment escalation plans, and ensuring high-quality, evidence-based care is implemented, evaluated, and tailored to the needs of both patients and those important to them.

What we’re looking for:

·       Registered Nurse on NMC Register

·       Experience in a relevant care setting, ideally in palliative/End of Life care

·       Be person-centred and compassionate when caring for the person and those important to them

·       Ability to develop and evaluate care management plans alongside other healthcare professionals that meets the complex needs of the person, and those important to them

·       Proven understanding of the barriers and enablers to good communication and how challenges can be overcome

·       Ability to remain calm in stressful situations

·       Flexibility to provide care in the area with the most need.
