An exciting and challenging opportunity has arisen for a fixed-term position within the Integrated Independence Team for an innovative, enthusiastic and experienced administrative assistant to be a Clinical Screener within the Integrated Independence Team single point of access.
We are looking for a Band 4 Assistant to coordinate the initial screening of all referrals into the service, as part of the Single Point of Access team. The role would also be well suited to an experienced member of a community therapies referral/co-ordination hub (or discharge hub). In doing this you will work as part of a strong multidisciplinary team, within an interdisciplinary working model, to facilitate hospital discharge and prevent acute admission.
Main duties of the job
Main duties include:
- Together with the team of Clinical Co-ordinators, coordinate the referrals into the Single Point of Access team, screening all non-complex referrals and allocating cases to appropriate care pathways and IIT colleagues
- To provide first point of contact assistance/advice for referrers, health professionals and service users, forwarding callers to Single Point of Access team Clinical Co-ordinators as appropriate
- Liaising with primary, secondary, health and social care providers, to facilitate service users into appropriate Integrated Independence Team pathway or declining referrals where necessary
- Assess clients referred as appropriate to decide on appropriateness for the service
- To manage an individual caseload of service users with provision of therapy assistant assessment, treatment and liaison with Integrated Independence Team colleagues for advice or support as required.
- To update all electronic systems in conjunction with Single Point of Access Clinical Co-ordinators including referral email account, spreadsheets and RiO system.

Essential criteria
- GCSE or equivalent
- Evidence of continuing professional development
Skills / Abilities
Essential criteria
- The ability to interpret clinical information, use analytical and problem solving skills to inform decision making.
- Coordinates and manages the caseload including prioritising the workload, appropriate use of escalation plan, time management and delegation
- A competent level of IT skills including user knowledge of core Microsoft Office packages, ECDL or equivalent
- Converse with and listen to the views of service users and their families
Desirable criteria
- Developed clinical assessment and rehabilitation treatment skills
Experience
Essential criteria
- Experience of work in the NHS or Local Authority or experience in an equivalent health care provider
- Experience of working in a multi-disciplinary environment
- Experience of working with people with rehabilitation needs
- Experience of working in a client centred, goal orientated way
Knowledge
Essential criteria
- Knowledge of rehabilitation services health and social care needs
- An understanding of the implications of cultural difference for service delivery
- An understanding of the NHS and Social Care, especially community based services
- Knowledge of promoting independence
