An exciting opportunity has arisen within the Lewisham Borough Based Continuing Health Care Team for a Complex Case Management Nurse Assessor.

 

The candidate must be a Registered Nurse. The successful candidate will be responsible for the case management of patients, and the provision of individual commissioning arrangements for individuals meeting the eligibility criteria for NHS Continuing Healthcare.

 

The post holder will be responsible for the assessment of care needs including a client group  of Young Physically Disabled  and Learning Disabilities, and be able to develop., implement,evaluate and review programmes of care.  This will additionally include the administration of processes and to be involved in Personal Health Budgets.

 

This role is based in Lewisham but will be expected to travel to out of borough to review placements if appropriate.You will be expected to have experience in NHS Continuing Healthcare (CHC) and has the skills to work as a senior member of the Continuing Healthcare team, with staff who are highly motivated and skilled nurse assessors. 

 

Main duties of the job

You will be expected to promote evidence-based best practice and provide clinical expertise to decision making processes regarding continued eligibility and funding, have excellent assessment writing skills that portray the holistic health needs of patients.

 

You will be able to manage your time and work in a pressurised environment.  You will have relevant experience in complex case management and drawing on previous skills, knowledge and be confident about what is required to deliver an excellent CHC service.  You will need to demonstrate a high level of interpersonal and communication skills, including the application of tact and diplomacy in challenging expectations and complaints, to liaise effectively with colleagues, health and social care professional, patient’s family members and others.
