Job overview

An exciting opportunity has arisen for a Band 4 Community Health Care Support Worker to join our Heart Failure inequalities project  team and Heart failure Specialist Service at Whittington Health. The project involves supporting patients who may face challenges engaging with healthcare professionals specialising in Heart failure. Within this role the post holder will support registered healthcare professionals to deliver  Heart failure care. There will be elements of patient contact, ensuring that the administration involved in care delivery is completed and supporting in facilitating some group consultations as delegated by the Specialist teams

Main duties of the job

  • To work within a multi-professional team and provide support to a team of health care professionals
  • To support specialist nurses in clinic settings and on home support/case management which may involve domiciliary visits to patient’s homes. 
  • To be responsible for administrative support within the team as required

Detailed job description and main responsibilities

  • To work within a multi-professional team and provide support to a team of health care professionals
  • To communicate effectively with patients and carers, using motivational interviewing skills where necessary.
  • To work autonomously within structured boundaries which are risk assessed and supervised by a registered healthcare professional
  • To ensure practice is evidence based, by keeping up to date with new research literature, and by regularly evaluating working practices through evidence-based practice projects, audit, and research.
  • To demonstrate compassion, empathy and sensitivity to clients who may have complex emotional and physical conditions and social concerns requiring moderate emotional effort daily.

Essential criteria
  • Education to GCSE level or equivalent Able to demonstrate competence in maths and English NVQ Level 2 OR 3 and relevant experience OR NVQ Level 4
Desirable criteria
  • Experience of community work in the health and/or social care field Comprehensive experience working in an area relevant to long term conditions. Awareness of local needs e.g., ethnic minority
