Do you want the challenging experience of a hospital pharmacy within a busy London based teaching hospital with plenty of training and development opportunities.

We currently have an exciting opportunity for a enthusiastic pharmacist  or current trainee pharmacist qualifying this year to join our rotational team providing an operational and clinical pharmacy service to meet the demands of a busy hospital environment at the West Middlesex site of Chelsea and Westminster NHS Foundation Trust.  You will gain experience in all aspects of hospital pharmacy, with rotations in clinical areas such as Medicine, Surgery, Paediatrics, Cardiology, AMU, Technical and Cancer Services, Patient Services and Older People.

You will be provided with the necessary supervision and training in each rotation.  There may be an opportunity to undertake the Joint Programmes Board/ UCL School of Pharmacy Post Graduate Diploma in General Pharmacy Practice for which West Middlesex , Chelsea and Westminster NHS Foundation Trust is an accredited centre.

1.  To effectively participate in the provision of clinical and operational pharmaceutical services to Chelsea and Westminster NHS Foundation Trust, West Middlesex site, under appropriate supervision for grade and experience, rotating through various specialist areas on a 4 monthly basis

2.  To participate in on call, weekend and shift working on a rotational basis and participate in 7 day working when required.

  • Patient Services
    • Provision of legally required supervision and validation aspects of the dispensing process
    • Dispensing for inpatients  and outpatients (including clinical trials and controlled drugs)
    • Checking technician/ assistant/ trainee pharmacist dispensing
    • Counselling outpatients to defined local, national and legal standards
    • Support management of stock control, dispensing workload, dispensary staff and contributing to the maintenance of a quality service that relates to clinical governance issues 
      • Provision of legally required supervision and validation aspects of the dispensing process
      • Dispensing for inpatients and outpatients (including clinical trials and controlled drugs)
      • Checking technician/ assistant/ trainee pharmacist dispensing
      • Counselling outpatients to defined local, national and legal standards
      • Support management of stock control, dispensing workload, dispensary staff and contributing to the maintenance of a quality service that relates to clinical governance issues
      • Participates in service audits as required
      • Effectively contribute to the production of Standard Operating Procedures (SOPs) for the relevant section
      • Undertake specific projects in accordance with objectives set
  • Clinical Pharmacy ServicesThe pharmacist will rotate through a range of clinical areas, including medicine, surgery, care of the elderly, paediatrics and cardiology, along with oncology and haematology as part of their Technical Services rotation.
    • Provision of ward/ clinical pharmacy services to the wards of the Trust to defined local and regional standards.  Identification of pharmaceutical problems of patients and application of therapeutic knowledge gained from clinical training programmes  and experience as a practising pharmacist.  This includes:
      • Ensuring that inpatient prescriptions are safe, accurate, legal, clinically appropriate for the individual patient, cost effective and compliant with local prescribing policies.  Recommending changes to the prescriber to ensure the above.
      • Advising other professional staff on medicines management and optimisation  issues.
      • Participating in clinical intervention systems and other quality monitoring initiatives
      • Implementing local formulary recommendations/ therapeutic guidelines and other related support to Divisional/ Specialist pharmacists
      • Implementing risk management and other Trust medicines related policies
      • Participation in clinical training schemes including timely attendance and  completion of course work and assessments
      • Analysis of all medicine charts for patient on allocated wards
      • Prioritisation and resolution of pharmaceutical problems involving all other healthcare professionals when necessary
      • Responsibility for the supply of prescribed medication to patients on allocated wards  in a timely manner
      • Responsibility for the review of the stock holding of medication on that ward with the nursing staff on the ward in order to ensure that there is an adequate amount and range of medication on the ward
      • Review prescriptions for accuracy and legality and identify actual or potential problems
      • Ensure the effective supply of medicines suitable for use as an inpatient and appropriately labelled for discharge if applicable
      • Ensure the effective outcomes of treatment with medicines
      • Monitor patients for potential and actual adverse effects of their medicines
      • Support the planning of patient discharge with respect to medication, including the transcription of requests for dispensing and timely discharge of patients in line with Trust requirements
      • Demonstrate professional accountability to patients
      • Participation in developing medicines management systems and auditing the outcomes of these developments
      • Responsibility for ensuring that all medicines (including controlled drugs) are stored according to requirements (including three monthly stock checks)
      • Undertake training in specialist areas as service and development needs arise
  • Acute Medical Unit
    • Participate in the provision of a clinical and dispensary service to the Acute Medicine unit including the duties outlined above
    • Participation in late and weekend shifts to the Acute Medical Unit
  • Technical and Cancer Services
    • Participation and accreditation in all aspects of technical and cancer services including:
      • Checking and screening for oral and intravenous chemotherapy
      • Checking and screening accreditation for adult and neonatal total parenteral nutrition
      • Checking and screening accreditation  for extemporaneous products and small numbers of prepacks
      • Quality control systems in line with EC Good Manufacturing Practice and risk management principles
      • Clinical pharmacy services to haematology/ oncology, as well as legally required professional supervision of pharmacy staff and validation aspects associated with technical and cancer services dispensing 
  • Additional/ Other 
    • Deputising for relevant senior staff
    • Participation in weekend/ public holiday and on call rotas
    • Participation in late duties
    • Participation in performance review and appraisal and identification of own learning and development needs
    • Participation in audit and project work in line with service development
    • Assisting in training and supervision of pharmacy technicians/ assistants and trainee pharmacists
    • Promotion of safe, economic and effective use of medicines
    • Ensuring that a written record of Continuing Professional Development (CPD) is maintained for review at appraisal
    • Attendance and participation  in departmental meetings and participation in departmental quality initiatives
    • Follow legal, ethical, professional and employers' codes of conduct
    • Demonstrate awareness and commitment to the Trust's Clinical Governance Agenda
    • Any other relevant duties as directed by Senior Pharmacy Managers
