Job overview

This is a physical health needs service

To work as part of the Community Treatment Team (CTT) to avoid unnecessary admissions to the acute hospital settings (BHRUT Hospitals). To assist with delivering nursing and therapy care as part of a multi professional service/team, within the accident and emergency and community setting working with patients towards rehabilitation, self-management, and independence throughout the continuum of care. Including being professionally and legally accountable.


Using current evidence-based therapy techniques, when based in the Accident and Emergency (A&E) setting, assist with providing a thorough and rapid assessment, facilitating a quick and effective discharge home. Engage with community services to create a seamless journey for patients from the A&E setting to home.
When based within the community undertake visits and holistic assessment of needs and assist with ongoing management of personalised care thus reducing the need for an acute admission.


The service is staffed 365 days per year from 8am till 10pm, the post holder will work various shifts during this time at any of the bases.
To assist with management of a defined caseload of patients with complex needs.

 

Main duties of the job

• To be responsible for organising and prioritising, allocated delegated workload on a day-to-day basis.
• To have organisational knowledge relating to Trust protocols and procedures and adhere to them.
• To ensure effective risk management at team level by accident/incident reporting.
• Carrying out and monitoring treatment programmes as directed.
• To identify and report any changes in patient care needs or changes in physical or emotional wellbeing to the lead nurse.
• Patients who have a wide range of physical disabilities as well as cognitive, perceptual, and mental health problems.
• To maintain accurate records in the patient’s RIO records within 24 hours, record new problems as they are identified and report all actions taken to the lead nurse.
• Adopt and MDT approach, ensuring patients are treated holistically.
• To support the CTT in the provision of therapy assessments and treatments.
• To undertake specialised nursing/therapist tasks following completion of theoretical training and acquisition of competency under the supervision of the therapy/nursing staff.
• To supply, fit and demonstrate the use of specialist equipment to patients and carers after appropriate training in consultation with a qualified therapist/nurse.
• To maintain confidentiality in all dealings with patients and their records.
• To offer support and teach patients, relatives and other carer’s appropriate skills, imparting knowledge to promote positive health.

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust.   All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.  As part of the process, new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes.  The induction will be held at our head office in Rainham, Essex.

High-Cost Area Supplement - London

This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436).
