Job overview

NELFT would like to introduce the launch of the Intensive Support Team, which aims to support people with a Learning Disability and Autism who requires specialist crisis intervention. We believe that individuals who display challenging behaviours, including those with a mental health condition, should have access to the same opportunities to live satisfying and valued lives.

The service was developed in response to the best means of supporting our people with a learning disability and autism at a critical time in their journey, offering the least restrictive environment and ensuring that we provide the best response to those users of the service who needs specialist support, together with their carers and families.

The post holder will work collaboratively with other LD and mental health care professionals to provide high-quality, comprehensive and dynamic specialist nursing services to maintain and improve the health and well-being of adults with learning disabilities. This will include working directly with service users who have complex healthcare needs, supporting other staff who provide clinical care to people with healthcare needs of lesser complexity, and also engaging in a range of health promotion activities.
The candidate will also be required to work with multiple agencies and service providers across NELFT, which will need excellent communication skills.

  • Member of the multi-disciplinary team and active participation in team meetings.
    • Have a key role in working with a wide range of health professionals and other partners and agencies to reduce health inequalities and promote access to mainstream health services
    • Work as partners with individuals and their families, providing advice and support, anticipating health needs, promoting self-care and enabling people to be as independent and involved as possible
    • Represent the service and individual service users at multi-disciplinary meetings, taking a lead role where appropriate and integrating speech and language therapy needs into a service user management plan.
    • Communicate often complex information effectively to service users and carers, using alternative methods of communication where appropriate, to maximise treatment potential whilst ensuring an understanding of risks and benefits of assessment and treatment/interventions.
    • Provide advice and teaching to service users, their families and carers, health professionals within the team and NELFT, the wider health community, and public, voluntary and private agencies.
    • Share information effectively and concisely for a range of situations and contexts to ensure safety and continuity of care Leadership.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust.  All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.  As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focussing in engagement, health and wellbeing and key processes.  The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - Outer London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,313 to a maximum of £5,436 p.a. pro rata for part time).
