The post holder will provide specialist clinical pharmacy support to frailty services at St Georges Health and Wellbeing Hub, including Frailty Virtual Ward. They will deliver medicines-related care to patients, support the provision of medicines management services, participate in education and training and lead on service improvements.

The post is a new position and is an exciting opportunity for an ambitious and driven pharmacy technician looking to develop their skills and support the expansion of integrated pharmacy services across frailty at the New St Goerges Health and Wellbeing Hub in Hornchurch.
They will work closely with the lead Pharmacist for Frailty and out of hospital care to further innovate and develop the frailty service at St Georges with the aim of reducing attendances to the emergency departments, reduce acute admissions into hospital and support our patients to get the most from their medicines.
The post holder will be responsible for supporting high-quality medication service to frailty virtual ward. The post holder will be heavily involved in the day to day running of the virtual ward, working collaboratively with a large, varied group of staff with all aspects of the medication service provided to patients.

Patients across north east London are benefitting from two new state of the art theatres at KGH and our Community Diagnostic Centres at Barking Community Hospital and St George’s Health and Wellbeing Hub will significantly increase the number of scans that can be carried out.
We’re looking forward to introducing an electronic patient record next year. This will mean the records of any patient visiting one of the seven hospitals run by BHRUT and Barts Health will be accessible to the clinical teams. It’ll make things easier for staff and will be better for patients.

Experience/ Knowledge

- A solid grounding in all aspects of hospital pharmacy practice.
- Experience of working in frailty and care of the elderly services with special emphasis on patient counselling and medicines reconciliation.

- Demonstrable use of clinical audit and quality improvement methodologies to improve practice.
