Applications are invited from enthusiastic candidates who are interested in the opportunity to be part of the Neurology Nurse team, providing high quality care to patients with a diagnosis of Parkinson’s disease living in The Hillingdon area. This is an excellent opportunity to work across both hospital sites and build links with Community colleagues.
This is a One year Fixed term post, and the appointee will join the existing multi-disciplinary team to support patients to self-manage their condition helping to avoid hospital admissions where possible. The post holder with provide specialist advice and education to both patients/carers and other teams within the hospital and primary care settings. There will be opportunities to contribute to innovation and ongoing development of the service.
This post would suit a Nurse with a sound knowledge of neurology, who is interested in contributing to the delivery of care to this group of patients .The post holder will have a good working knowledge in the diagnosis and management of Parkinson’s disease and be able to demonstrate confident clinical assessment skills and have experience working as an autonomous practitioner.

Main duties of the job
Assessing Parkinson's patients, offering support and expert guidance to patients and their carers
Working with the Multi disciplinary team to support complex patient care
Medication management and reviewing medications ,making recommendations as pts condition changes
Facilitation of timely PD review to assist with In patient management and discharge planning for PD pts
First point of contact to help manage this patient cohort

Detailed job description and main responsibilities
- Have specialist skills in the care of neurological patients including the co-ordination and delivery of treatments and evaluating patient management.
- To be a core member of the MDT and actively participate in MDT meetings and ward reviews.
- To ensure effective two-way communication with patients, carers and other healthcare professionals regarding patient care whilst supporting equality and diversity at all times..
- To respond to the changing needs of the NHS with positivity and creativity.
- To identify risks within the service, report accordingly and identify appropriate treatment plans to negate risk.

Essential criteria
- NMC Registered Nurse Level 1
- First degree in Nursing
- Highly developed specialist knowledge of Parkinson’s and Neurological conditions
- Evidence of ongoing CPD
- Evidence of teaching in a variety of environments
- Advanced clinical assessment skills and has the ability to implement these into practice
Desirable criteria
- MSc level qualification (or working towards it)
- Post Graduate Qualification in specific speciality or equivalent experience
- Non-Medical Prescribing qualification
- Teaching and Mentoring qualification

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
