CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting! 

 We are seeking enthusiastic senior nurses/paramedics to join our Early Intervention Vehicle (EIV) Service, delivering care through the Unplanned Care Team.

The purpose of the EIV is pathway is to deliver crisis management to patients living in Herts Valleys & prevent hospital admission, providing  clinical assessment by a nurse/paramedic jointly with staff from Herts County Council. 

The EIV pathway is closely aligned to other Unplanned Care services, which deliver high quality clinical care to patients in their own home at times of crisis.  The purpose of the Rapid Response (RR) pathway is to prevent hospital attendance/admission and the Discharge to Assess (DTA) pathway is to facilitate early discharge from hospital.  The EIV will supplement and support the delivery of Rapid Response and Discharge to Assess in the Unplanned Care Service, often referring patients onwards after assessment. 

The EIV delivers care to patients across all four Herts Valleys localities, responding to non-life-threatening priority calls.  The practitioner, will carry out a holistic physical & functional assessment, providing any necessary urgent equipment to ensure they remain at home & identify further needs- making the necessary onwards referrals.

Main duties of the job

The successful candidate will be a registered nurse or paramedic with advanced clinical skills, significant knowledge and experience in the clinical care of adults and older people in a range of healthcare settings and the management of long term conditions.

You will be self-motivated with strong organisational skills and able to make complex clinical decisions quickly and autonomously as required. As a senior clinician you will use your advanced clinical expertise, levels of judgement, discretion and decision making to support patients at home, improve patient outcomes and reduce likelihood of unnecessary admissions to hospital. You will be required to complete holistic assessments, giving appropriate interventions as required and have knowledge of community services to plan the on-going care of patients on the pathway.

Detailed job description and main responsibilities

The post holder will work autonomously and in collaboration with other health care professionals, statutory and voluntary agencies and the RR team to deliver excellent standards of patient care for patients requiring support in a crisis.

To respond rapidly and deliver high quality, skilled clinical assessments to patients at their usual place of residence, providing emergency equipment and assistance where required.

The service operates through pre-rostered shift patterns spanning 8am – 8pm 7 days a week (subject to extend once fully established).

To utilise specialist skills and knowledge to holistically assess needs.

To work, as required, on a flexible basis to cover all roles required within the team including RR team if required.

Qualifications

Essential criteria
  • Registered Nurse with current NMC registration OR Paramedic with HCPC registration
  • Evidence of recent and continuing post-registration professional education and training equivalent to post-graduate diploma level
Desirable criteria
  • Experience of working in Primary Care/ Accident & Emergency
  • Experience of assessing and providing quality care to people with Long Term Conditions
  • Qualifications in Community and/ or Accident and Emergency health care
  • Qualification in teaching or practice based learning
  • Physical Clinical Assessment Skills Course or willingness to undertake the course
  • Independent prescribing qualification

Experience

Essential criteria
  • Evidence of recent substantial post-registration professional clinical experience
  • Extensive range of clinical skills and experience including, e.g. IV therapy, venepuncture, cannulation, rehabilitation
  • Experience of working within a multi-disciplinary team
  • Experience of supervising junior staff and co-ordinating a team on a day to day, regular basis
  • Experience of undertaking comprehensive health needs assessment and writing care plans
Desirable criteria
  • Experience of implementing evidence-based care in practice

Skills and Knowledge

Essential criteria
  • Ability to communicate clearly and effectively, in both verbal and written format
  • Basic IT skills including use of Microsoft Office packages and email
  • Principles of care for patients with long term conditions, relevant NSFs and other national standards
  • NMC/HCPC Codes of Professional Conduct and application to practice
  • The principles of clinical governance and application to practice
  • Clinical supervision – purpose and application
