Trosolwg o'r swydd

Working autonomously, be responsible for promoting a patient centred pathway to expedite discharges and reduce delayed transfers of care within Integrated Medicine. 

Working with Complex Discharge Team, Medical escalation leads, ward managers and discharge practitioners to expedite delays in discharge.

To attend all routine and emergency bed meetings, to give and feedback on escalations. To liaise with Head of Nursing and Matrons daily at the safety brief.

Be part of the review and implementation of the LNWUH NHS Trust discharge policy within all integrated medicine wards, proposing and working towards service changes within the multi-disciplinary teams or outside own specialist area.

Work in close partnership with all hospital colleagues including all members of the multidisciplinary team to maximise ensure smooth but safe  flow of patients through the hospital from ED to the wards and onwards to the appropriate destinations.

Oversee the team of flow facilitators, coordinating the healthroster and ensuring cross site cover under supervision of the matron and meeting the e-roster KPIs

Management and leadership of the associate (band 5) flow facilitors, ensuring appraisal and MAST training is completed appropriately.

The role is predominantly NPH based but you may need to work cross site to ensure there is cover and support for associate flow faciliators

Communicate, establish and maintain relationships, influence and gain the co-operation of others by communication with people in a manner that is consistent with their level of understanding, culture and background, as well as communication differences such as hearing loss, speech difficulties and preferred language.

 Negotiate with all other divisional escalation leads to ensure that all escalations are acted upon and resolved in a timely manner. Provide information and attend the weekly stranded patients meeting. This meeting requires you to liaise without outside agencies.

 Excellent communication skills to overcome difficult and highly emotive atmospheres, such as Full escalation and internal incidents where additional beds need to be located and all staff are expected to feedback about delays to escalations and discharges of patients at senior meetings.

Be aware and implement the “Safeguarding Adults” criteria, raising staff awareness and supporting all involved in the process. This will demand a high level of knowledge so that you can provide support to the ward discharge co-ordinators.
