Senior Staff Nurse-Transplant Outpatients (6 month secondment)

Guy’s and St Thomas’ is among the UK’s busiest and most successful NHS foundation trusts. We provide a full range of hospital and community services for people in south London and as well as specialist care for patients from further afield including cancer, renal, orthopaedic, respiratory and cardiovascular services.

Guy’s is home to the largest dental school in Europe and a £160 million Cancer Centre opened in 2016. As part of our commitment to provide care closer to home, in 2017 we also opened a cancer centre and a kidney treatment centre at Queen Mary’s Hospital in Sidcup. St Thomas’ has one of the largest critical care units in the UK and one of the busiest emergency departments in London. It is also home to Evelina London Children’s Hospital.

Evelina London cares for local children in Lambeth and Southwark and provides specialist services across south east England including cardiac, renal and critical care services. We lead a number of specialist service networks aiming to ensure children are treated locally where possible, but have access to specialist expertise when they need it. Our community services include health visiting, school nursing and support for families of children with long-term conditions. 

Our adult community services teams deliver care at the heart of the local communities we serve, working in partnership with GPs, local authorities and other healthcare and voluntary sector organisations. Working with our partners in Lambeth and Southwark, we are focusing on new ways of working to improve care for local patients.

In February 2021 the Royal Brompton and Harefield joined Guy’s and St Thomas’ NHS Foundation Trust, bringing together world-leading expertise in the care and research of heart and lung disease. Our merger provides a once in a generation opportunity to build a lasting, world-renowned heart and lung centre, providing the highest quality care for patients and conducting world-leading research.

We have a reputation for clinical excellence and high quality teaching and research. We are part of King’s Health Partners, one of eight accredited UK academic health sciences centres. In partnership with King’s College London we have dedicated clinical research facilities including an MHRA accredited Phase I clinical trials unit.

We are seeking a highly motivated individual who has excellent care values, a strong sense of teamworking who is keen to develop their skills in a new specialty, which delivers high quality patient centered care.

The Transplant Out-Patient Department delivers a range of outpatient services for a growing population of our pre and post- cardiothoracic transplant patients. This post will involve providing a wide range of clinical care and support to Transplant patients and their family and will also include administration duties.

The successful candidate will need to be flexible in their working practice and possess good communication and interpersonal skills. Previous Transplant experience, management of complex patients, proven clinical and management skills relevant to the out-patient environment are desirable.

In return successful applicants will be fully supported whilst gaining a wealth of experience in the care of the pre and post cardiothoracic transplant patient as well as working very closely with the multidisciplinary team. Not only will you be able to deepen and broaden your experience, but you will also benefit from excellent training and developmental opportunities with support from a dedicated team.

Royal Brompton and Harefield hospitals joined Guy’s and St Thomas’ in February 2021 and is the largest specialist heart and lung centre in the UK and among the largest in Europe. We provide treatment for people with heart and lung disease, including rare and complex conditions, offering some of the most sophisticated treatment that is available anywhere in the world.

Our integrated approach to caring for patients from before birth, through childhood, adolescence and into adulthood and old age has been replicated around the world and has gained Royal Brompton and Harefield an international reputation as a leader in heart and lung diagnosis, treatment and research. 

Harefield Hospital (HH) is a regional centre for cardiology and cardiothoracic surgery, and an international centre for adult heart and lung transplantation. It is one of a small number of UK cardiac centres assisting in development of implantable mechanical ventricular assist devices in the management of end-stage heart failure. It also provides a primary intervention service for acute coronary syndromes to selected Trusts and the London Ambulance Service, in outer West London and the Home Counties.  It has approximately 1,185 staff, 180 beds with 5 operating theatres, and 4 catheter laboratories.

Main tasks & responsibilities:

Patient/customer care (both direct & indirect) 

 · Assess, plan, implement and evaluate programmes of care, actively seeking evidence-based practice which will improve the quality of care and meet the needs of the patient.

 · A holistic approach to caring should be planned that will include the appropriate level of physiological, pathological, spiritual, socio-economical and cultural aspects which influence the care of clients within your specific speciality.

· Implement a nursing plan for an assigned group of patients, indicating assessment and re-assessment formulated for individual pathways.

 · To act as the patient advocate in line with the NHS plan.

 · In partnership with other professionals provide excellent patient care prioritising workload according to changing clinical need.

· Ensure that nursing documentation is maintained to the Trust standard (including electronic records).

· Participate in conducting nursing audit to achieve the agreed standards, ensure CQUIN targets are met.

· In conjunction with the Transplant Outpatient team contribute to developing and maintaining a departmental ethos and a standard approach to the delivery of excellent nursing care.

 · Maintain excellent clinical standards of nursing practice.

· To regularly take charge of the clinical area, utilising resources effectively and to ensure that safe staffing levels are maintained for their shifts.

· In conjunction with the Departmental Manager provide a suitable and safe environment for the physical and psychological well being of staff, patients/ clients and their relatives/carers.

· Under the supervision of the sister/charge nurse contribute and participate to the process of setting goals and benchmarking standards of care. Collaborating with other members of the Transplant Outpatient team to ensure that patients needs are met to a consistently high standard.

· Promote awareness and compliance amongst colleagues regarding Trust policies, procedures, guidelines, standards, core behaviours, the 6C’s, and the Trust Nursing Strategy.

· To actively participate in the Trust’s framework of clinical governance and patient safety walkabouts within the ward or department area.

· To provide a suitable and safe environment for the physical, psychological and spiritual wellbeing for staff, patients/clients and their relatives/carers. Page 3 of 8

· Identify and report clinical risks and report to the ward/departmental Sister/Charge nurse/modern matron so that appropriate action can be taken in accordance with the Trust risk management policy.

 · Record and report all incidents/complaints involving staff, patients and visitors in accordance with Trust policies and assist in any investigations as required.

· Address verbal complaints immediately with the aim to resolve at local level.

· To regularly take charge of the clinical area, utilising resources effectively. Ensuring that safe staffing levels are maintained for their shift.

· To delegate tasks and activities to a range of team members in relation to patient care, for example when taking charge or as link nurse/champion.

· To actively promote a healthy lifestyle for patients and staff in line with national strategy requirements.

Communication 

· To demonstrate politeness, respect, courtesy and sensitivity in dealing with patients/clients, visitors/relatives and colleagues - maintain good customer relations.

· Maintain and promote effective communication with all members of the multidisciplinary team and other wards/ departments.

· To adapt communication skills to meet the needs of patients who may have barriers to understanding e.g. language

· Contribute towards sharing good practice within the ward/department and Trust wide.

· To promote a positive image of the Trust always.

· To work cohesively with all members of the Transplant Outpatient team in ensuring that the very best services to patients are provided at all times. People management

· Take responsibility for own annual leave in line with Trust and local Policy and Procedure

· Participate in the Trust’s Appraisal and Personal Development Plan schemes by meeting with the team leader/appraiser at least annually, which includes feedback from a selection of internal staff and team members.

· Participate in the programme for preceptorship and be a mentor as required.

· Maintain a personal, professional profile, identifying role and skill developments in line with the Scope of Professional Practice. Resource management

· To work together with the Transplant Outpatient team to ensure effective use and maintenance of resources and adequate stock levels.

· Participate in planning the duty rota taking into consideration the needs of the service and in conjunction with the parameters set by the department Sister/ Charge Nurse. Page 4 of 8

· To actively engage with rostering in the Transplant Outpatient department and adopt a flexible approach to personal rostering to ensure the needs of the service are met.

Service development 

· To contribute to the development and maintenance of a positive learning environment for colleagues, patients and visitors.

· To participate in staff education and development, including orientation and supervision and act as a mentor and a resource for learners and the nursing team.

· Be aware of the service delivery/business plans. Policy development

· Under the supervision of the department Sister/Charge Nurse to work on delegated projects relating to local policy development.

· To be actively involved in the implementation of Trust policies and procedures at ward / department level, reinforcing their use. Information management

· Participate in the collection of audit and research data as required.

· Use data which will contribute to the improvement of patient care.

· To have an active Trust intranet and email account and adhere to the Trust’s Information Services and Information Governance Policies

· Use the Trust internet to enhance care and for professional development.

· Undertake relevant training for electronic information systems in place and under development.

· Following Trust/local guidelines to access results from the current electronic database systems.
