The postholder will work collaboratively within the Care Coordination Centre. The Care Co-ordination Centre is at the heart of Community Specialist Palliative Care services. It combines Rapid Response Team, Clinicians of the Day and the Triage and Deployment Team. Its role is to ensure that

Working for our organisation
North London Hospice has been at the heart of our community for more than 40 years, supporting more than 3,500 every year across the boroughs of Barnet, Enfield and Haringey.

Our hospice continues to play a crucial role in helping services work together to meet the needs of those in our community living with a variety of conditions. We’re passionate, caring and focused on ensuring that everyone who uses our services are cared for and supported at one of the most important moments of their lives. In addition, we also support people in transition between services, sectors, and specialisms.
To do this, we must ensure that all our services and teams continue to form vital strategic partnerships and we need to make sure we have the best possible leaders to deliver.
