Job overview

An opportunity has opened up for a talented individual to join the Community Forensic Learning Disability team as a Community Forensic Practitioner . This community team is part of the Forensic LD Pathway, which also includes Medium and Low Secure wards.

It provides care and support for service users living in the North Central London area.  It provides a combination of case management of restricted individuals, co-working with local community LD Teams, It also provides a wide range of local training and risk assessment expertise to local community teams, accommodation providers and local services. There is also an opportunity for inpatient forensic LD  in-reach work.

The successful applicant will work within a multi-disciplinary team, ensuring nursing contribution to the team's functioning.  They will have strong clinical skills and will carry a small caseload, acting as care coordinator for patients discharged from secure settings. They will be required to work proactively with the in-patient service to enhance the discharge process.

 The post holder will have an input into providing training to local providers on areas such as risk assessment and Positive Behaviour Support. They will ensure that the team delivers on its KPIs and will liaise closely with the performance managers to ensure that data is appropriately collated and reported.

Main duties of the job

Ensure the care of service users is implemented according to their care plan and to the patient's individual needs.

Work as a member of the multi-disciplinary team 

Manage a defined caseload and fulfilling the role of Care  Coordinator.

Organising CPA and C(e)TR  and will provide co-produced care plans.

Applying appropriate skills of prioritisation, decision making and implementing interventions, taking  account of clinical risk factors and personal safety. 

Apply assessment , interpretation and judgement skills in assessing and managing clinical risks.

The post holder will report directly to the Team Manager and will work in partnership with members of the multi-disciplinary team.

Qualifications

Essential criteria
  • Registered RNLD/RMN
  • ENB 998 or teaching equivalent

Experience

Essential criteria
  • Experience of working within a Forensic setting
  • Experience of working with people with Learning Disabilities/Autism
Desirable criteria
  • Experience of managing a caseload

Knowledge

Essential criteria
  • Detailed Knowledge of the Mental Health Act 1983
  • Ability to implement policies and procedures, leading their implementation where appropriate
Desirable criteria
  • Evidence of involvement in QI projects and service improvement
  • Able to evidence current and practice knowledge of clinical and policy issue related to the clinical speciality and the wider NHS
