Job overview
We are seeking to recruit an enthusiastic and motivated person to join our service of Community Paediatricians and Physician Associate in providing care for children and young people (CYP) with additional needs and supporting their families /carers.
You will be a core member of the community paediatric multidisciplinary team and we work to deliver high quality, proactive service aimed at improving the outcomes of our service users and this involves working within the multi-disciplinary team at Carmelita House and services around the children and young people, including General Practitioners, Early Start Teams, Social Services, Special Educational Needs Department and voluntary agencies.
Main duties of the job
The Specialist Nursing role require individuals to have or develop expertise in the speciality, be able to teach and support staff in the clinical areas, undertake audit or quality improvement projects as required and to gain patient feedback to review and improve services. You should be an effective communicator, motivated and self-directed and willing to continue to develop professionally.

Ealing Community Partners (ECP) is a collaboration of providers that runs Ealing community health services by Ealing CCG. It is led by West London NHS Trust, and partnered with Central North West London who employs the post holder.
The community paediatric services in Ealing are based in Carmelita House, the home of Ealing Services for Children with Additional Needs (ESCAN). This is a joint Health / Local Authority venture and is one of the few in the country to host specialist community children’s services, children’s mental health services, Education and children with disabilities social work team within one building and one managerial entity. Clinics are also run at other locations across Ealing, and in specialist schools and nurseries.
Ealing is a vibrant residential suburb of West London and is the 4th largest borough in North West London with a population of approximately 340,000 people, of which 67,000 are children aged 0-15. Ealing has a strong migrant population with approximately 40% of the Ealing population being non-UK born.
Detailed job description and main responsibilities
Job description:
· To be point of contact for the Community Paediatric Team for families/carers, other professionals within ESCAN and other agencies. This will imply particularly to children with Down syndrome.
· The post holder would have opportunity to participate in other clinics depending on their area of interest and would be supported where training is required. Possible area of interests are;
a. Neurodevelopmental team – participating in autism pathway particularly post diagnosis
b. Involvement in sleep pathway for CYP requiring specific support
c. Carrying out comprehensive health needs assessments of children and young people with complex needs within different aspects of the overall Child Development Service, for example: carrying out developmental assessments, taking detailed neurodevelopmental histories, carrying out school-based observations.
d. Neurodisability – participating in multidisciplinary clinics for children with health needs such as Cerebral Palsy.
· Attend relevant meetings, as required, including safeguarding and child protection meetings, team business meetings and specialist meetings e.g. Down syndrome pathway, Autism pathway, as needed.
Please note this is not an exhaustive list as the role may evolve over time according to the needs of the local population and the post-holder's career development. Training can be provided if needed.

- Community Specialist Practitioner, RNLD, RNChild
- Evidence of Post registration education to degree level.
Experience and knowledge
Essential criteria
- Substantial post registration community experience
- Experience in key working and care coordinating
- Experience of liaising/negotiating with other professionals and inter-agency collaborative working
- Up to date clinical practice
- Understanding of current health legislation and priorities
- Child protection experience
Desirable criteria
- Working knowledge of special educational needs and SEND reforms
