Main duties of the job

We are seeking a Community Matron within the South Beds Localities to work within the Complex Care/Primary Care Home Teams providing long term condition and minor illness assessments for patients that meet the service two-hour response criteria.You will be a strong leader, focused on achieving real improvements in outcomes for patients and will be confident in your ability to support, coach and develop individuals and teams to deliver consistently high quality services.

You will be innovative, forward thinking, self-driven with excellent communication and interpersonal skills.  You will be an experienced Registered Nurse with top to toe assessment and independent prescribing skills with a background in minor illness or chronic disease management.

Detailed job description and main responsibilities

You will take pride in your work and be highly motivated and dynamic, have a flexible approach and be able to embrace change. Being a collaborative worker and the ability to network and build relationships with a diverse group of stakeholders is essential.

You must have a driving license and have access to a vehicle in order to carry out your duties.  You will be required to work a shift pattern covering 7 days a week from 08:00-20:00.

Education/Qualification

Essential criteria
  • Registered General Nurse
  • Demonstrate evidence-based practice and up-to-date clinical skills
  • Independent prescriber
  • Relevant teaching/mentoring qualification and evidence of teaching skills

Experience

Essential criteria
  • Significant/recent experience working at a high level (Band 6 or above)
  • Experience of clinical leadership
Desirable criteria
  • Community Experience

Knowledge and Skills

Essential criteria
  • Evidence of managing long term conditions/minor illness
  • Knowledge of the principles of health promotion and prevention of illness
  • Knowledge of the principles of Safeguarding of Vulnerable Adults and Children
  • Evidence of and commitment to multi-professional working
Desirable criteria
  • Knowledge of Local Services

Other

  • Demonstrate an understanding of national government initiatives in health and social care and how they impact on local services
