Job overview

The Transfer of Care Hub (TOCH) is a co-located, hospital-based team consisting of multi agency services to coordinate the discharge of patients from Croydon University Hospital to the appropriate setting.

The cornerstone is the ‘Integrated discharge team’, (IDT) which has a wide range of skillsets and backgrounds including nursing, therapy, and social work. The team co-ordinates all aspects of supported discharges and is the single point of discharge from CUH.  It includes roles where professional boundaries have been blurred, to streamline processes and improve experience for patients and staff.

 

 You will be primarily responsible for operational and strategic oversight and management of the TOCH and continuously developing it in line with Croydon system’s vision and National Hospital Discharge guidance and good practice.

 

You will be highly experienced in hospital discharge and intermediate care, with the knowledge and authority to manage stakeholders from the entire system and to make effective use of varied discharge pathways to ensure residents receive the right care, in the right place at the right time. This will include arbitrating on disputed cases involving highly complex and sensitive circumstances.

2. Continuously building and developing the TOCH in line with the Croydon system’s vision and National Hospital Discharge guidance and good practice.

These responsibilities will require both managing staff and working with stakeholders from across health and social care to understand the requirements for effective service delivery based on the needs of the Croydon population and the supporting services. As a system appointment, the Transfer of Care Hub Lead will work closely with colleagues across organisational boundaries in Croydon (CHS, Croydon Council, and the voluntary sector) and will work in a variety of locations including Croydon University Hospital and multiple community sites.

 

The post-holder will be highly experienced in hospital discharge and intermediate care provision, with the knowledge and authority to manage stakeholders from across the entire system and to make effective use of discharge pathways across the system. This will include arbitrating and acting as a final decision maker on disputed cases involving highly complex and sensitive circumstances.
