We are looking for an enthusiastic and dynamic individual to join the Integrated Neighbourhood Team. The INT Senior Case Manager will work with core community-based MDTs  and wider professional network to develop integrated neighbourhood/care teams. They will proactively undertake case finding, manage, co-ordinate and support patients with multiple complex long-term conditions using the whole systems integrated care approach.

 

The post holder has the overall responsibility for overseeing core MDT operations and ensuring that the developing INTs.  This includes building relationships with Primary Care Networks (PCNs) and key stakeholders on the wider determinants of health to improve outcomes and quality of life and prevent unnecessary admissions to hospital.

 

The post holder will support core team members to ensure patients with complex needs on the community caseload are receiving optimum health and social care support. The role is approximately 60% clinical . You will work flexibly across the team, and will ideally have experience operational change management as well as good knowledge of social care and/or health delivery.

 

 As the role supports partnership working, good communication and inter-personal skills are fundamental, alongside strong technical ability. You will use your experience to ensure a structured management process of methodologies and principles to deliver positive outcomes and measurable results.

The key responsibilities include:

1.    Responsibility for the operational management and professional clinical leadership of INT functions. This includes supporting Huddle meetings on GP and/or Neighbourhood level.

2.    Oversee the performance of the core teams, delivering against agreed objectives and target outcomes within available allocated resources

3.    Oversee the coordination function that supports the existing community base services with forming multidisciplinary teams, delivering care via MDT meetings and enabling closer working across Health and Care Localities teams and Primary Care Networks

4.    Oversee the core co-ordination function and support alignment and integration across the teams as part of service delivery. Y

5.    Work closely with the Integrated Management Team and existing service leads, to focus on testing and embedding innovative solutions to transform service delivery and work towards improving outcomes for residents.

6.    Responsible for exploring current practice, identifying areas for improvement, and providing professional and expert input into development, and planning of the appropriate change using current evidence to evaluate outcomes.

7.    Ensure the quality of patients’ experience and lead on facilitating changes designed to improve patient experience.

8.    Support the development of care pathways that deliver on partner objectives and ensure excellent patient outcomes and experience.

  • Case Management of people with LTC module

Experience

  • Evidence of post registration education e.g. specialist practitioner, nurse practitioner, post, registration cousperses or portfolio of evidence
  • Primary Care experience in care of people with long term conditions
  • Experience of working in Community services
  • Experience of clinical audit, benchmarking and evaluation of services
  • Experience of interagency and collaborative working

Skills
