This a community health services physiotherapy role, full time Band 6 role

Are you a physiotherapist with a passion for delivering exceptional community and evidence-based therapy and rehabilitation; supporting clients to remain safely and independently within their place of residency? Bedfordshire Community Therapy Service is looking for a part time Physiotherapist to work within the community therapy team to support individuals within the Discharge to Assess Beds, Inpatient Rehabilitation Beds or within their own place of residency within the community (home).

You will prevent hospital admissions and support timely discharges by promoting high standards of rehabilitative intervention which will support a client’s personalised program for strength/confidence building, supporting clients to return home if possible or supporting decision for their onward care when required... You will also have the chance to support residential and care homes across the county to create an environment that promotes self care and as much independence as possible with their residents.

You will represent Bedfordshire Community Therapy Services within a Multi-Disciplinary Team and at system partner meetings to discuss your caseloads, observations, visions for improved care to meet the needs of the local population. We encourage new ways of thinking and working in order to provide timely and successful results for our clients.

Swydd ddisgrifiad a phrif gyfrifoldebau manwl

-        Responsible for leading on and providing evidence based therapy assessments, interventions and rehabilitation programs to promote an individual’s safety and independence.

-        Identify concerns or risks that would impede an individual’s safe discharge to their place of residency after a hospital admission or a short stay within a D2A or IPR Bed. Using problem solving skills, clinical judgement and collaborative working with system partners to resolve the concerns for the benefit of the client.

-        Work with the D2A service manager to improve D2A pathways and processes related to therapy services as required

-        Provide advice, equipment and evaluations based on a client’s individual needs.

-        To be responsible for the oversight of ongoing therapy intervention and reviews post discharge.

-        To work as an autonomous professional according to Professional Standards and the Health and Care Professions Council’s regulations.

-        To supervise, educate and assess performance of physiotherapy students. This would be to a graduate level and involve working with the universities to ensure the standard of practice and teaching meets the standards set by the degree level qualification.

-        Work without direct supervision; to hold responsibility for own caseload and supporting the management structure in place to promote the provision of an excellent service.

-        In times of system surge or under system pressures you may be required to support other areas of therapy services

To provide cover for the service 7 days a week as part of a rota.

This role requires you to travel across the county, so you will require a full UK driver’s License and access to a vehicle. This role is part of a 7 day service via a rota and so flexibility to work weekends and bank holidays is also required.

  • Practice Educator trained and willing to support Student placements
  • Graduate degree in Physiotherapy

Experience

  • Broad range of physical experiences clinically
  • Supervision/Appraisal experience
  • Willing to give presentations/lead teaching sessions for colleagues
  • IT Skills (word, e-mail, excel, virtual platforms)
  • Experience of working within the NHS as a physiotherapist

Knowledge and Skills

  • Detailed understanding of a wide range of medical conditions. .
  • Knowledge and understanding of mental health conditions that affect physical conditions
