A very exciting and unique opportunity has arisen to join two innovative and creative teams in SLaM on a fixed term basis for 18 months. The Croydon Intensive Support Team (IST) is a small multidisciplinary team that works with people with a learning disability at risk of admission or placement breakdown.
The Transforming Care Nurse role is a unique and integral part of both teams, supporting people to remain at home in their local community and avoid unnecessary admission but also support those who have been admitted to be safely discharged back into their local community.
You will provide specialist enhanced clinical assessment, a small caseload of patient Your role will be to review the patients’ care pathway and progress with the hospital’s MDT, and to plan and facilitate hospital discharge where appropriate. In IST your role will be to assess, plan and manage the physical health care needs of patients who are engaging in behaviours that challenge and require intensive support to reduce their risk of hospital admission or placement breakdown.
You will be expected to work as an autonomous practitioner to provide advanced assessment, clinical decisions and co-ordinate complex and challenging situations. You will be supported to advance your skills and experience and take on leadership projects and roles within your area of expertise.

- The post holder will have responsibility for supporting the quality of clinical practice across therapeutic interventions for service users with learning disabilities, mental health problems and complex/forensic needs.
- To work closely with and provide increased levels of support to family carers and clients’ support network where needed.
- To work in a person-centred way and promote service users’ social inclusion.
- To practice within Applied Behaviour Analysis/ Positive Behaviour Support frameworks and promote the development of the Intensive Support Team
- To aid the development of local capable environments in provider services through offering specialist support and advice

- Formal training in Positive Behaviour Support or Applied Behaviour Analysis
Experience

- 3 years of working with vulnerable/complex clients
- Experience of working with families and a complex support network in highly emotive contexts
- Experience of multi-professional collaboration at differing levels including partnership working with other statutory and voluntary agencies.
- Experience of working in a multi-cultural environment and knowledge of methods to develop culturally sensitive practice
- Experience of working with other client groups and of using a variety of models of care including PBS and ABA
Knowledge / Skills

- Specialist clinical knowledge of difficulties faced by clients with complex needs, including evidence based practice.
- Working knowledge of all relevant legislation/guidance including MHA, CPA procedures, Human Rights Act, NSF, Child Protection and NHS plan.
- Knowledge of evidence base specifically PBS/ABA, NICE guidelines on CB and national policy, specifically the Transforming Care Agenda.
- Ability to present ideas with clarity and in a persuasive and influential style.
- Ability to communicate effectively both verbally and in writing.
- Ability to negotiate and reach compromises in verbal discussions in a manner which inspires confidence and respect from others.
- Ability to build and maintain high standards of professional links and communication channels with a variety of personnel and agencies.
- Ability to analyse complex issues/problems, gather relevant information and exercise sound judgement in reaching the most appropriate conclusions.
- Ability to manage own time effectively and produce pieces of work on time and to a high standard.
- Ability to organise and plan ahead effectively.
- Recognition of own limitations, strengths and weaknesses and an ability to seek advice when necessary.
- Ability to easily gain the trust, confidence and respect of service users, their relatives and staff at a variety of levels and disciplines
- Innovative and creative and a willingness to try something new.
- Methodical approach to work with attention to detail.
- Basic computer skills.
- Highly motivated and able to work independently.
