Job overview

This role is to support families with infants, toddlers and children, providing 1:1 and group support to early nutrition, play and preparing  for school readiness. 

You will need to travel to a variety of sites across Barking and Dagenham,   working in partnership with  Family Hubs and voluntary services 

This role will support families with children with topics including, supporting breast feeding and early nutrition  introducing solids , fussy eating,  healthy sleep routines , temper tantrums and play .

Main duties of the job

  • Offering specialist support and guidance for families to establish health eating habits whilst  maintaining breastfeeding – face-to-face in group settings, at home, or on the phone/video call.
  • Support families with with toddler behaviours Including co-delivery of evidence-based parenting interventions (working with Best Start for Life team).
  • Provide group and individual sessions around transition periods in the early years – such as introduction to solids, toilet training, sleep, and oral health
  • Promote healthy transitions service effectively – identifying opportunities and proactively suggesting ways for families to connect with the local community.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary  period or currently a bank member of staff). 

High Cost Area Supplement - London

This post attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735 per annum pro rata).

 

Detailed job description and main responsibilities

  • To work in partnership with the 0-19 Healthy Child Programme team, Best Start in Life Team and Family Navigators to ensure families receive an integrated service during the early transitional years
  • To record accurately in the Health Record and other professional records as appropriate under the direction of the Community Public Health Nurse, ensuring that these are kept up to date
  • To complete referrals to other agencies in liaison with the Community Public Health Nurse
  • To input information on the RIO system
  • To provide developmental screening of children as requested by concerned carers/parents and professionals in liaison with the Community Public Health Nurse
  • To plan, prepare and provide health information for children / young people, carers and staff.
