Transitional Care Flow Hub Lead.
0800hrs to 2000hrs covering a 7 day service.
Are you a people person, caring and want to make a difference? If yes then this could be for you. An exciting opportunity is available for a Registered General Nurse, qualified Physiotherapist, Occupational Therapist or paramedic to work for Lincolnshire Community Health Services (LCHS).
The Transitional care and flow lead role is a permanent role focusing on the Flow post discharge both within the hospital and also as part of the admission avoidance. The LCHS team (both Assertive In Reach and Care Transfer Hub within the acute) work closely with the Acute hosptials and also within the community to facilitate - A&E, UTC, SDEC, Fracture Clinic and alongside our other community services and stakeholders based within the community and acute. This is a new role which is pivotal in preventing admission to the hospital and facilitating appropriate fast and easy discharges once medically fit within the acute hosptials ensuring patient needs can be safely met back in the community with the emphasis on home first principle.

To work as an accountable and autonomous practitioner, leading clinically in effectively discharging and assisting in admission avoidance within Lincolnshire’s hospitals and community as well as also working reactively to the daily changing situations presented from the acute and the community.
Recognising and responding to requests for patient reviews in a timely manner enabling a rapid continuation of flow back to the community and to avoid admission to hospital. As well as pro-actively identifying potential patients for admission avoidance, working to the "HomeFirst" principle, leading the team. To be able to work alone and also as part of a MDT ensuring the patient stays centre of any decision and plan. Ability to liaise with other key stakeholders and services to provide the most appropriate support/onward needs of the patient.
Ability to think outside of the "box", have influencing skills and ability to lead discussions. To demonstrate well informed clinical decision making in order to provide support and advice to patients, carers and other members of the multidisciplinary teams.
Prepared to work flexibly to meet the needs of the service and respond in a timely responsive way.

for, using well-informed decision making and problem solving skills. Undertake medicine management review for patients.
To apply for this role you must hold current registration as a Registered health care professional. You should hold a degree level qualification or be able to demonstrate equivalent experience. Minimum two years experience in a relevant setting essential. You must have evidence of leadership, development and effective relationship skills.
The Integrated Urgent and Emergency Care services in Lincolnshire see thousands of patients every year both in the acute setting and community. As well as providing a urgent response, we are responsible for the management of patient flow across our system, working with partners from a range of other organisations to deliver care closer to home.
