The Base will be Sleaford however we try and keep you within Locality and after the Assessment you can either work from the office or work from Home.  Team Meetings are held in the office once a month, and we would look for you to come into the office if your assessment for the day is cancelled. 

NHS Lincolnshire ICB is seeking to recruit a highly motivated, enthusiastic and forward thinking clinical assessor to work as part of our Continuing Health Care team. The successful candidate will be working across our varied and diverse population.  We are recruiting to fill posts in the East and South of the County.  You need to be able to work autonomously and collaboratively within a multi-agency environment and dynamic skill-mixed team.

The CHC Case Manager will ensure that the health care needs for Continuing Healthcare (CHC) are identified and met in accordance with the National Framework.

The post holder will ensure that they adhere at all times to the relevant National Framework and guidance, NMC Code of Professional Conduct and work within established policies, procedures, protocols and systems ensuring quality standards and performance.

Required for the role:

• Registered Nurse RGN, RMN or RNLD or Allied Health Professional with qualification.
• Knowledge of the CHC framework and its application
• Knowledge of NHS and social services guidance.
• Strong knowledge base around safeguarding children and adults
• Evidence of teaching/mentorship
• Clinical knowledge base in order to ensure comprehensive holistic assessment for continuing care
• A background that can demonstrate clinical experience in a healthcare setting including inter-agency and multi-agency working
• Evidence of continued professional development
• Competent in IT including good typing skills. Able to work with a database, Excel, Microsoft
