We have an exciting opportunity to recruit 1 FTE practitioner to support with patient flow across the acute and community. The opportunity is at Lincopln County Hosptal with times supporting Grantham and Pilgrim hosptial and the Transtional care beds.

The purpose of the role is to support with triaging and facilitation of discharge for patients  on Pathway 1 D2A and pathway 2. 

Prif ddyletswyddau'r swydd

The practitioners based at Lincoln County will be part of the integrated discharge hub to support in triaging and influence pathway decision making for those medically optimised patients within the acute and influence Flow. The post holder will oversea band 5s, band 4s, band 3s, and band 2s across the service and will be called upon to lead the service remotely when on shift at times.
