This is a great opportunity for a pharmacist with hospital and/or primary care experience to join the LLR Care Homes Pharmacy Team.

This unique role involves working across the interface in LLR at UHL admissions wards, in primary care, and at care homes.

Our team offers specialist support for care homes and has a particular focus on medicines optimisation for frailty, polypharmacy, and “high-risk” care home residents. The work involves specialist medication reviews, medication audits, MDT working, support for care home projects around Leicestershire, and much more.

Successful applicants will get the opportunity to be involved in innovative projects, research, and help shape the national agenda for care homes pharmacy.

Job Overview

  • To provide a medicines optimisation service to care home residents across LLR and work across the interface to ensure the best outcome for these patients.
  • To deliver a clinical service to patients, including taking patient drug histories, rationalising prescribing, advising on and compiling treatment protocols, therapeutic drug monitoring, and monitoring compliance (in primary and secondary care).
  • To support the CMG Lead Pharmacist in maintaining and improving the pharmaceutical service to the clinical management group (CMG).
  • To be responsible for the delivery of an agreed standard of pharmaceutical care to patients to achieve the safe and cost-effective use of medicine.

 

The vacancy is for 4.5 days per week but we are happy to consider other working patterns.

Prif ddyletswyddau'r swydd

  • Participate in the delivery of optimum pharmaceutical care to patients served by the clinical directorate on the specified wards, identified care homes, and across the interface
  • Provide advice to patients, medical and other clinical staff on any drug element of relevant guidelines and to MDT care teams, including areas of highly complex medicines and chronic disease states to optimise patient care
  • Contribute to the management of the relevant drugs budget via regular expenditure analysis feedback and participate in structured ongoing drug use audits to ensure optimal drug usage
  • Collate and feedback intervention and audits as determined by senior pharmacists
  • Provide education and training to all other healthcare staff as appropriate within UHL and across LLR
  • Actively pursue audit, research, and development within the area and the department. Publicise the results of such work through presentations and publications
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
  • Liaise as appropriate with other health care professionals within primary care, particularly providing a focus on care across the interface and within Care Homes
  • Work to identify patients within Care Homes who are at risk of admission into hospital and minimise this risk by effective medication review
  • Undertake other such duties as commensurate with the post
