UHL Community Diabetes team is entering an exciting phase of expansion in our nursing team would welcome community clinical aides in diabetes (CCAD) to come and share the journey with us.

This vacancy will be based at the LGH site.

Diabetes Community Diabetes Team is changing the way we work. Reviewing people with diabetes now includes the use of technology as part of their normal care. This has enabled the community DSN team to deliver care tailored to the individual and aim to reduce the need for hospital admissions.

Due to the diversity of patients supported across LLR with diabetes the post offers a unique and varied role to enhance patient care, you will be expected to travel across Leicestershire depending on the needs of the patients and will work alongside the community DSN team. You will need to have a valid driving license and the ability to safely drive if required for travel for job-related duties such as patients own home or district hospital clinics

Main duties of the job

UHL community diabetes service works Monday to Friday. Your duties will include

  • To provide support to patients to help them make informed decisions about their care. This support can be offered through face-to-face, telephone discussions, or by responding to email queries.
  • To provide individualised diabetes care tailored to patient needs and circumstances.
  • To provide diabetes education to patients, covering topics such as hypoglycaemia management, insulin initiation teaching
  • To ensure smooth running of district hospital clinics by supporting clinicians in various duties.
  • To support patients to download their glucometer data before their appointments with the team
  • To liaise with patients regarding annual health checks to improve patient safety
  • To support diabetes team with health promotion. This includes sourcing promotional materials.
  • To provide education to patients on how to use blood glucose and ketone meter.
  • To ensure education materials are accessible to patients. Source supplies for the delivery of care.
  • To support the diabetes community team in delivering Freestyle Libre or Dexcom education sessions

Detailed job description and main responsibilities

You will be working with a passionate and dedicated team in the specialism of diabetes. We pride ourselves in providing excellent and individualised care. You will be working within a small team of Community Diabetes Specialist Nurses  working Monday to Friday currently. You will be expected to ensure your provide patient comfort, reassurance and continuity of care.

  • GCSE - A-C in English and Numeracy or key skills level 2

Experience

  • Previous HCA role 2 years or more
