Passionate about supporting cancer patients?

We are looking to recruit a band 3 Cancer support worker to join our Upper Gastrointestinal (GI) Cancer Nurse Specialist Team.  You will work closely with the cancer nurse specialists and the wider multidisciplinary team to provide care and support to patients and their families who have been diagnosed with stomach and oesophageal cancers.  

We are a friendly and supportive team who will support you to develop your skills and expertise and provide opportunity for you to network with our highly valued group of cancer support workers who work with our specialist site specific cancer nursing teams. 

Previous experience in cancer care is desirable however we welcome candidates who are passionate about providing  quality care

As part of our team you will help support our patients through their cancer pathway.  You may liaise with service users over the phone, virtually or face to face. You will need to be able to demonstrate good communication, interpersonal, organisational and IT skills and be able to maintain effective communication with the Registered Nurse/health care professional regarding issues relating to assigned duties, promptly reporting areas of concern.

You will play a key role in the provision of personalised care and support plans for our patients through the use of holistic needs assessments.

Swydd ddisgrifiad a phrif gyfrifoldebau manwl

To triage messages and telephone queries within an agreed scope of practice.

 Signposting of queries as appropriate to the clinical, information or administrative teams.

 Ensure prompt documentation of patient/carer contacts in Somerset cancer database, recognising that the key worker has personal accountability for entries to patient records made by others under their supervision so must be informed.

 Using excellent communication skills, and appropriate tools and procedures, liaise as appropriate with the cancer nurse specialists regarding non-routine enquiries, referring  complex problems to the nurse specialist team for assessment and review.

 To support the cancer nurse specialist in coordination of care for cancer patients from referral throughout their treatment making every contact count.

 To visit patients as in-patients or out-patients as directed by the CNS providing information and support as directed.

 Support patients by coordinating and assisting in the provision of holistic needs assessments and development of personalised care and support plans at pivotal points along their cancer pathway.

 To provide a ‘touch point’ for patients to ensure two-way communication during treatment.

 To provide guidance, contact and support for cancer patients as they journey through our hospital(s).

 To ensure all relevant patient appointments for nurse led services are notified in a timely manner providing an excellent patient experience providing required information.

 Support people affected by cancer to self-manage during and after their cancer treatment.

Coordinate face to face appointments as per multi-disciplinary team’s instructions.

Support and contribute to audit processes, governance, research, clinical research trials and service development.

 Support the planning/inviting/organising of informative events for patients

 Plans own workload on a daily basis and sets priorities for workload management.

 Able to complete tasks in a timely manner and according to relevant protocols for service

  • GSCE English or equivalent
  • • Relevant NVQ Level 3 qualification or equivalent including healthcare certificate
  • • Satisfactory level of General Education

Knowledge and Experience

  • • Previous experience as a health care assistant/clinical aide role in a relevant setting
  • Evidence that they have an understanding of the role and responsibilities of a Support Worker in further information section
  • • Demonstrates ability to pass on knowledge to others
