An exciting opportunity has arisen to take on the role of Transfer Practitioner within the Adult Critical Care Coordination and Transfer Service (ACCOTS). 

ACCOTS is a newly commissioned service for the Midlands region. It is hosted by the University Hospitals of Leicester and provides coordination, triage, decision support and transfer of critically ill patients between hospitals for escalation of treatment, repatriation or capacity reasons.

This is a fantastic opportunity to become part of a new team and service. Rotational opportunities are considered.  As a Band 6 Transfer Practitioner, following completion of the appropriate additional training, you will assume responsibility for the specialised care required to transfer critically ill adults. 

You will be based at ACCOTS base at Castle Donnington but travel will be required to UHL sites on an ad hoc basis. Shift work is required to meet service needs.

By working collaboratively with the multi-professional team, the post
holder will act as a professional role model, setting standards of practice
and promoting clinical effectiveness and actively participating in practice
developments identified through the service governance processes.

You will be expected to support and mentor new members of the multiprofessional team, demonstrating an active role in on-going education programmes. In partnership with the Midlands Critical Care Network, the post holder will support the relationship between the Service and referring centres through taking on the role of link practitioner for  designated hospitals; assuming responsibility, with the Lead Practitioner and Band 7 Educator, for the delivery of outreach education and annual updates.

Duties to also include:

  • Transferring of Level 3 Critical Care patients as part of a skilled multi-disciplinary team

  • On occasion  in the future transferring as an independent practitioner of  Level 2 (high dependency) and Level 1 (ward level) patients.

  • Working collaboratively with the multi-disciplinary team

  • You will be expected to support and mentor new members of the multi-disciplinary team, demonstrating an active role in on-going education programme.

  •  Ensure all aspects of performance, attendance, training and appraisal are addressed in accordance with Trust policies.

  • To participate and support staff in the development of service delivery audits and implementation of findings to improve patient care.

  • Maintain efficient and effective communication with all members of the transport multi-disciplinary team.

  • To meet regularly with Lead Practitioner and Clinical Lead to discuss professional progress and development within the transport team.

  • Act in a Professional manner and ensure self and transfer staff adhere to the UHL Uniform policy at all times.

  • Act as a positive role model through the provision of effective professional leadership for ACCOTs with demonstration of competent and accountable clinical practice.

  • Act as an adviser/educator and role model to less experienced doctors, nurses and therapists (MDT) to ensure appropriate care is planned, implemented and evaluated

  • Demonstrate knowledge of current research associated with all aspects of the speciality and advise others where appropriate on the implementation of relevant research findings.

  • In partnership with other members of the multi-professional Team develop guidelines and protocols to maintain the provision of high quality critical care.

  • Demonstrates excellent verbal communication and listening skills with patients, relatives and all the multi-professional members of staff.

Training and Qualifications

  • Appropriate current registration with NMC / HCPC
  • ILS Competent
  • Basic and Advanced life support (APLS/EPLS) or willingness to undertake
  • Evidence of ongoing, dynamic continuing professional education within a critical care environment.

  • Successful completion of a recognised critical care transport course (adult)
  • Successful completion of recognised Post Registration qualification in Critical Care or equivalent.
  • A recognised teaching and assessing qualification including mentorship course
  • Successful completion of 60 Credits post graduate learning.
  • Completion of Steps book 1,2,3

Experience

  • Minimum 3 years recent Post Registration experience
  • Recent experience working within an Adult Critical Care Unit with 3 Years post reg experience
  • Portfolio of evidence showing achievements in practice / speciality
  • Understanding of professional and current issues in critical care transfers
  • Competent computer skills including Microsoft Office applications
  • Certified IV drug administrator, both centrally and peripherally
