Job overview

This post consists of a funded 2 year apprenticeship via HEE England completing MSc Advance Practice, commencing at Band 7.  On completion of the degree, progression to Band 8A will be awarded on a permanent basis.

In addition to the standard requirements for a post at this level (Nursing or AHP background), you must also have a minimum of 4 years' post-registration experience as well as at least a Grade C (or equivalent) in Maths & English at GCSE level. Please note that these additional requirements have been set by the University of Bolton. If you do not have these, you cannot be considered for the post as you will not meet the University's criteria for the training course.

We’re looking for an ambitious, enthusiastic, motivated and experienced clinician who will be required work as an autonomous practitioner identifying, assessing, diagnosing and treating older adults with frailty needs while taking the next steps in developing community services. If you have a strong interest in reablement, working with patients' strengths and promoting their autonomy by enabling safe discharge to place of residence to maintain their independence in the community, this could be for you.

This is a full time post (37.5 hours) working across Monday to Sunday between 0800-2000. This post cannot be offered on a part-time basis due to the taught element.

Main duties of the job

The Community Assessment Unit (CAU) consists of 21 beds including 9 single rooms. Its primary function is to operate as an older persons assessment unit, accepting patients for step up/step down care and safe discharge into the community. The role is to assess, diagnosis and treat patients independently referred from Accident and Emergency, North West Ambulance and the Community React Team and step down patients from the acute site, in providing the right care in the right place and ensure a safe and timely discharge for this cohort of patients in our six chair assessment area and to take an active role in the management of the 21 bedded ward area alongside the wider MDT.

Detailed job description and main responsibilities

Professional & Clinical Practice

  • Ensure clinical practice reflects all current NMC documents including (a) Code of professional Conduct (b) Standards for record keeping (c) Administration of
  • Work within the trust’ s policies and procedures and the parameters set down in agreed local clinical protocols and guidelines and national best practice guidelines such as those published by NICE and within own sphere of competence. Trust policies, procedures and guidelines can be found on the trust website.
  • Assess a range of patients with undifferentiated and differentiated problems and under supervision if needed and work towards the development of a comprehensive management plan in undifferentiated and differentiated
  • Develop and utilise advanced knowledge and skills to assess, examine, diagnose, plan, treat and refer patients if needed to meet the patients physiological, psychological and social needs and in response to the changing needs of the
  • Utilise appropriate investigations to confirm diagnosis and action as
  • Work towards organising the clinical management of patients, utilising advanced knowledge and skills prioritising patients accordingly. This may be through triage, face to face and telephone consultations, MDT meetings as appropriate using professional clinical
  • Recognise and respond appropriately to urgent and emergency situations with the support and guidance from other team
  • Work towards managing situations that are unfamiliar, complex or unpredictable and seeking to build confidence in others with the support and guidance from other team members. This will involve utilising clinical judgement and decision making to initiate appropriate interventions and treatment for patients.
  • Make and receive referrals to and from other health care professionals
  • Communicate patient’s condition, verbally and in writing as required, to other members of the multi-professional and multi-agency team and contribute towards the decision-making processes in patient management.
  • Discuss with mentor / supervisor the utilization of medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based
  • Work towards completion of the Non- Medical Prescribing qualification (V300) as agreed in the MSc Advanced Practice. .

Responsibility for Health & Safety
