An exciting opportunity has arisen for a band 3 member of staff  to join our enthusiastic multi-disciplinary team. The candidate will work closely with service users in ways which promote independence, to improve existing skills and to acquire new ones and maintain dignity.

The Integrated Urgent Care Team is a joint health and social care service which is leading on the implementation of ‘Home First’ which is Tameside and Glossop Admission Avoidance and Discharge to Assess Model.

Applications are invited from enthusiastic and motivated people who have experience of delivering care.

Shifts are carried out over a seven-day rota including weekends and bank holidays,  shift patterns are  07:30 - 15:30 or 14:00 -22:00.

 Please note that this service is based in the community and travel will be an essential part of the role and access to a vehicle will be essential to carry out the role.

Main duties of the job

The main purpose of this role is to ensure that safe care is provided to patients within a community setting i.e. an individual’s own home.
The post holder will work within the Integrated Urgent Care Team supporting both qualified and non-qualified nursing, therapy and social work staff in the safe management of patient care.  Co-ordinating individualised patient care and quality. Maintaining trust standards in documentation and support audits undertaken in the community.
The post holder will work in the Integrated Urgent Care Team and will support all multi-disciplinary team members in monitoring and maintaining policies, standards of care, staff and service development within the department/ward environment.

Detailed job description and main responsibilities

Ensure patient safety is maintained.

Actively promote best practice, ensuring all care is evidence based.

Work closely with service users in ways which promotes independence, to improve existing skills and to acquire new ones and maintain dignity.

Participate in audits of practice, research and clinical effectiveness in accordance with the Divisional audit plan.

Support nursing and therapy procedures and treatments in accordance with approved procedures.

Escalation of any concerns and feeding back progress to Team Leaders.

 Maintain accurate and contemporaneous records and ensure records are clear, concise and legible.

Actively assess and manage risk within the immediate area in accordance with Health and Safety.

Develop the skills and knowledge necessary to prevent and control infection.

Works as both a team member and without direct supervision where appropriate.

Show commitment to personal education and professional development, to promote a culture of life long learning.

Safeguard at all times confidentiality of information relating to patients and staff.

Technical Care Instructor- Community Based:

Undertake personal care tasks including:

Personal hygiene, including washing and bathing.

Dressing/ undressing, including helping to get up and go to bed.

Helping to use the toilet and dealing with continence problems.

 Helping with movement

Helping with personal appearance.

Carry out personal exercise programme as directed by professionals

Be professionally responsible for adherence to and abide by Trust policies and procedures.

Assisting service users in taking/using medication.

Working with service users in settings other than their own homes when required by the care plan – e.g Day Care activities, communal activities in sheltered housing.

Be aware of and respond to individuals and carer’s emotional needs and to support people in distress.

Undertake Practical tasks which may include:

Preparing meals in line with dietary needs and requirements. 
Shopping and signposting or support with budgeting if required.

Cleaning personal items including spectacles and dentures.

Work through and become competent with all expected aspects of the TCI role.

Deliver and access with equipment, when competant

The post holder may be required to work in other areas in order to ensure business continuity and patient safety is maintained at all times.
