Job overview

This is an exciting opportunity to join a team of learning disability liaison nurses, supporting adults with a learning disability throughout their contact across our busy hospital site.  The post holder will be based at Royal Bolton Hospital and will work in partnership with learning disability nursing colleagues and the wider multi-disciplinary team.  The learning disability liaison nurses provide expert advice and support to ensure adults with a learning disability receive excellent inpatient and outpatient care. 

We are looking for a qualified learning disability nurse with experience of working with people with learning disabilities in a community or hospital setting.  The post holder will have excellent communication skills, good organisational skills and an excellent understanding of the Mental Capacity Act. 

Main duties of the job

You will be expected to:

Support adults with a learning disability to navigate hospital services, working in partnership with colleagues to ensure the appropriate application of reasonable adjustments. 

Help to identify, and overcome, potential barriers to good health and positive patient experience.  Ensure hospital processes are followed to maximise health outcomes, including the use of hospital passports, reasonable adjustment care plans and formal planning processes/meetings. 

Support teams to make clinical decisions following assessment and contribute to individualised care planning. 

Assist colleagues in determining a patient's ability to give informed consent, leading on Mental Capacity Act processes where indicated.  This includes the application of MCA best interest process, you will be skilled at organising, attending and chairing formal meetings. 

Work collaboratively and negotiate with adults with learning disabilities, carers and others across the health and social care sector to support individual case management, resolving any areas of conflict or unmet need and generating possible solutions.

Support the development of service policies and processes to enable the ongoing improvement of services for adults with learning disabilities. 

Collate and present data as required to report on patient outcomes, participate in benchmarking exercises,  inform quality improvement plans and to support audits.

We have a clear, robust supervision process and all staff have access to support and guidance from their supervisor and the wider team as required.  You will work in partnership with your supervisor to assist in the development of new skills.  The post holder will be subject to a probationary period which allows for additional support as required. 

The Trust is committed to training and team members are supported to consider future development needs and opportunities via the appraisal process and via regular contact with the supervisor and learning disability matron.

Detailed job description and main responsibilities

The post is a fixed term band 6 role, working alongside our existing learning disability liaison nurses.  As a core member of the team you will have the following responsibilities:

Direct support to adults with a learning disability admitted to the Royal Bolton Hospital or attending one of the outpatient wards/departments.  This includes providing staff with expert knowledge and advice to ensure smooth admission and maximise health outcomes. 

Working as part of a multi-disciplinary team, contribute to individual care planning, helping to identify and overcome, potential barriers to healthcare. 

Support colleagues in the correct application of Mental Capacity act processes, organising and leading meetings as required. 

Work in partnership with adults with learning disabilities, their family members/carers and other health and social care professionals to ensure the delivery of a high quality service. 

Organise and manage a clinical caseload, prioritising support when required. 

Ensure hospital processes are followed as required to support admission and treatment plans.  Assisting in the development of new processes, policies and systems as required. 

Support colleagues working in urgent care to develop immediate care plans and support the implementation of these for unplanned hospital admissions/contacts. 

Develop and deliver training to health and social care colleagues as required, helping to ensure learning disability awareness across the service.
