Cuerden Ward is a busy acute, proactive area specialising in diabetes care but also for a variety of acute medical specialities. We are an established team and although it is a very busy and demanding ward we work closely as a team and deal with the day to day pressures with positivity and high levels of energy.

You will be responsible for providing a range of personalised care for patients and a range of clinical duties under the guidance and supervision of the Nurse in charge.

We love making a difference and working together as a close family to gain the rewards of making our patients journey enjoyable and successful. To do this, we are looking for Nursing Associates / Assistant Practitioners with a genuine interest in medicine to join us.

Experience of working with Medical patients in a hospital setting would be beneficial.

You will need to be flexible in your working hours to work shifts/unsocial hours including nights as required.

Contribute to the assessment, implementation and evaluation of care to meet patients’ needs in accordance with agreed local and national standards and evidence-based guidelines under supervision.

-Support and provide patient education and health promotion.

-Support the delivery and co-ordination of care, through appropriate professional practice.

-Contribute to the management of risk and maintain patient safety, conforming to health, safety and security legislation, policies, procedures and guidelines.

-Assist in setting, monitoring and maintaining standards of care to improve the patient experience and outcome of care.

-Communicate all relevant information to the multidisciplinary team internally and with external organisations as required, to ensure seamless care.

-Maintain accurate and complete records of all care activities, in a timely manner, adhering to local and professional standards for documentation.

-Act as a role model, presenting a positive image of self, team and the organisation.

-Promote equality in care and practice by recognising, respecting and meeting the needs and choices of individuals.

-Ensures environment of care is safe, well equipped and compliant with standards for hygiene and cleanliness.

-Maintain and develop own professional competence and skills within scope of role.

-Actively participates in the appraisal and Personal development planning process as reviewee.

-Proactively supports Trust wide or local changes and developments in nursing practice e.g intentional rounding, alcohol screening as directed etc.

-Actively try to resolve issues and complaints at local level and then report the outcome to line manager

Qualifications and Education

Meini prawf hanfodol
  • Foundation Degree in Health and Social Care OR Achieved LTHTR Assistant Practitioner Programme.
  • Maths & English grade 4-9 or grade C or above OR Functional Skills Maths & English Level 2.

  • Basic Life Support Cascade Trainer
  • Key Mover Cascade Trainer
  • ANTT Assessor
  • Good IT skills

Knowledge and Experience

  • Experience of working as a Trainee Assistant Practitioner within an acute hospital setting.
  • Proven competency of performing a core range of clinical procedures.
