Job overview

An exciting opportunity has arisen to appoint an inspirational, passionate and enthusiastic Nutrition Nurse Specialist to join our established nutrition team.

The post requires vision, innovation and the ability to work across professional boundaries, underpinned by a real desire to provide the highest standards of patient care. You will be an enthusiastic and motivated individual with advanced clinical skills who can work autonomously and as a key member of the nutrition team.

The post-holder will work with the Lead Nutrition Specialist Nurse, Lead Consultant and other members of the multi-disciplinary team to provide a specialist nursing service to patients requiring nutritional support, carers and health care professionals. As a key member of the nutrition support team the post holder will work in collaboration with the multi-disciplinary team in formulating care plans and policies in relation to nutrition support. A commitment to individualised evidence based patient care and ability to act sensitively with empathy as well as having excellent communication skills is essential.

Main duties of the job

The post holder will be expected to liaise appropriately with all key members of the multidisciplinary team (MDT) and service users.

The post holder will promote patient autonomy through education and support allowing the individual to make informed choices about their own nutritional care.

Demonstrate developed empathetic interpersonal and communication skills in dealing with patients and carers through the patient pathway.

Act as a resource to all members of multi professional teams working within the Trust and the community by giving advice and guidance on the management of artificial nutrition and medical devices.

Act as the patient’s advocate, conveying to the patient and their carers the options in terms of treatment/interventions. This may not always offer a positive outcome and will require the use of honesty and empathy.

Liaise with multiple internal and external teams to enable effective management of patients who require nutritional support.

Detailed job description and main responsibilities

KEY RESPONSIBILITIES

 Assessing patients’ suitability for enteral and parenteral nutritional support taking into account their physical, psychological and social needs.

Participate in the Nutrition Support Team activity caring for inpatients receiving short-term parenteral nutrition. This may include carrying out a period of study and becoming an Independent Non-Medical Prescriber and leading multi-disciplinary ward rounds.

Counselling patients considering artificial nutritional support with regards to Quality of life issues, Training required, Support available, Carer’s needs and Complications associated with treatment.

Supporting and training staff in the safe and competent care of patients receiving nutritional support.

Being available to visit wards and clinical areas to offer advice and support to staff, as required, on nutritional issues.

Troubleshooting, training, supporting and advising patients receiving parenteral and enteral nutrition.

Participate in the nutrition nurse led clinics and telephone helpline, directing care or admission where required.

Carry out patient assessment, review and following training carry out invasive technical procedures e.g. PEG removal, insertion of fine bore feeding tubes, changing of balloon gastrostomy tubes and insertion of nasal bridles.

Keep an accurate and up-to-date audit of the enteral and parenteral service provided, maintaining a database daily. Assist in the formulation of an annual report and data collection related to the service.

 MANAGEMENT

 Assist in the development of operational and strategic planning for the delivery of Nutritional services. Assist in delivering the Nutrition Nurse Service and Nutrition Support Team service within the Trust. Assist the wider multi-disciplinary team in the development of service standards.

Maintain accurate, contemporaneous, confidential and timely patient records ensuring that all patient documentation (both written and electronic) adheres to Trust Standards.

 TEACHING

Contribute to study days organized and run by the nutrition nursing team. This includes developing and delivering educational sessions and delivering presentations on all aspects of nutritional support. Provide advice, support and training on specialist nutrition practices, such as naso-gastric tube placement, in both formal and informal settings. Settings may include clinical ward areas and classroom sessions.

Assist in providing competency based, clinical teaching to junior members of staff.

Provide training to patients and carers in the management of their enteral feeding devices.

RESEARCH AND DEVELOPMENT 

Be aware of current trials and other studies being conducted within nutrition support and be able to inform colleagues and patients about these. Contribute to the research and audit activity undertaken by the nutrition nursing team.

Actively contribute to the clinical governance agenda within the Trust. This includes participating in risk assessment within the clinical area and assist in developing action plans for untoward incidents in conjunction with the Lead Nurse.

Assist the Lead Nutrition Nurse to identify new and innovative clinical practices that may improve patient care 

Participate in relevant committees and forums both locally and nationally e.g. the National Nurses Nutrition Group (NNNG)
