Job overview
We are seeking to appoint enthusiastic, motivated, forward thinking Band 5 Registered Nurses (or those due to qualify this year) to work within the Community nursing service.
Experience of working in a community setting is not essential as the successful candidates will receive a full supportive induction into this role. The post involves 7 day working which includes bank holidays. We are able to offer full and part time posts.
We would like to hear from any registered nurses and student nurses due to qualify, who have a genuine desire to nurse patients in their place of residence. We are looking for a good team player who delivers high quality nursing care, and is able to rise to the challenge of community nursing.
The Integrated Community Services Division includes Integrated Discharge, Intermediate Care, Neighbourhood’s (including Community Nursing), Falls and Frailty, Treatment Rooms, Diabetes, Endocrinology, Specialist Weight Management, Bladder and Bowel, Homeless support, Treatment rooms, Anti-coagulation, Community Neurology, Health Improvement, Allied Health Professionals and End of Life care.
Main duties of the job
To work as part of the 24-hour community nursing service, under the direction of the district nursing sister, providing high quality holistic nursing care to patients.
The staff nurse role carries delegated responsibility for the assessment, planning, implementation and evaluation of evidence based nursing to patients in their own homes and other community settings.
To assume delegated responsibility for the supervision of the team in the absence of the district nursing sister when required.

Qualifications
Essential criteria
- First level registered nurse
Desirable criteria
- Teaching/mentorship post-registration qualification
- Diploma in health related area or equivalent experience
- Non medical prescriber.
EXPERIENCE
Essential criteria
- Previous District nursing experience.
- Understanding of Community Palliative/End of Life Care
Desirable criteria
- Undertaking complex assessments and the planning and evaluation of care packages
SKILLS
Essential criteria
- Excellent communication both written and verbal
- Team worker
- Ability to work independently in community setting
- Motivated to self develop
Desirable criteria
- IT skills
KNOWLEDGE
Essential criteria
- Understanding of clinical governance framework and implications for practice
- Understanding of NMC standards and implications for practice
- Ability to problem solve and make clinical decisions
- High clinical standards
Desirable criteria
- Awareness of current NHS policy and organisational change
