Specialist Rotational Physio: Stroke Rehab and Community Neuro Team

37.5 hours per week

This is an exciting opportunity to work as a rotational specialist physiotherapist within Stroke and Neuro Rehab.. 

You will rotate through the Stroke Rehab Unit (including neurological out-patients and ward-based neuro patients) and in the community for the Community Neuro Rehab Team.

You will carry out physiotherapy assessments, develop therapy plans and deliver treatments in a variety of settings; these will include the hospital, patient home, clinic and other community settings. You will offer education and support to patients and their families.  

You will be expected to practice autonomously but work as part of an established, integrated multi-disciplinary team. You will be expected to carry a complex clinical caseload and be required to delegate work to support staff.

Section 1- Main Responsibilities

Professional/ Clinical

Perform advanced physiotherapeutic assessment of patients with diverse presentations and complex conditions and develop and deliver an individualised treatment programme demonstrating sound clinical reasoning.

To educate patients and carers to encourage self-management


To contribute to the MDT and work collaboratively within the wider health and social care economy. 

To be responsible for your own caseload and a defined area of service, working without direct supervision. Supervision takes the form of regular formal training and clinical reasoning sessions, peer review and case conferences.

To undertake all aspects of clinical duties as an autonomous practitioner.

Undertake evidence-based audit and research projects to further own and teams clinical practice within each rotation. Make recommendations to clinical team leaders for changes to practice. May lead the 
implementation of specific changes to practice or contribute to service guidelines.

SECTION 2- KNOWLEDGE AND SKILLS:

 

RESPONSIBILITY FOR PATIENT CARE: 

 

Be professionally and legally accountable for all aspects of own workload, including the management of your patient episode of care. 

 

Undertake a comprehensive assessment of patients including those with diverse or complex presentations/multiple pathologies; use advanced clinical reasoning skills and manual techniques 

 

Formulate and deliver individual physiotherapy treatment programmes based on a sound knowledge of evidence-based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills. 

 

Take delegated responsibility from the Lead Physiotherapist for managing patients with particular conditions to be responsible for providing specialist physiotherapy assessment and treatment plans for patients with these conditions within each rotation 

 

Formulate accurate prognosis and recommend best course of intervention, developing comprehensive discharge plans. 

 

Assess patient and carer’s outcome of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack the capacity to consent to treatment. 

 

Evaluate patient progress, reassess and change treatment programmes as appropriate. 

 

Have difficult and challenging discussions with patients and their families regarding patient prognosis, outcomes and expectations.  

 

Work in various clinical environments; these will include hospital ward, clinic, home and other community settings.

 

Be able to clinically supervise junior staff, assistants and students. 

 

HUMAN RESOURCES: 

 

Be responsible for maintaining own competency to practice through CPD activities and maintain a portfolio which reflects personal development. 

 

Be responsible for supervising students. 

 

Delegate work appropriately to junior physiotherapists and assistants. 

 

Maintain and develop current knowledge of evidence-based practice in the areas of each rotation, developing specialist knowledge of particular conditions and patient type. 

 

Participate in staff appraisal scheme as an appraise and be responsible for complying with your agreed personal development to meet set objectives and competencies. 

 

RESPONSIBLITIES FOR PHYSICAL AND FINANCIAL RESOURCES: 

 

To be responsible for the safe and competent use of all equipment, patient appliances and aids by patients and ensure that junior staff/assistants attain competency prior to use. 

 

COMMUNICATIONS AND LEADERSHIP 

 

Use a range of verbal and non-verbal communication tools to communicate effectively with patients to progress rehabilitation and treatment programmes.  This will include those who have difficulties in understanding or communicating, for example who may be dysphasic, depressed, deaf, blind or who may be unable to accept their diagnosis. 

 

To be responsible for maintaining accurate and comprehensive patient treatment records in line with the Chartered Society of Physiotherapy standards of practice.  

 

To be able to compose written advice to patients and carers and develop patient information leaflets, referring to Senior Physiotherapist when necessary. 

 

To liaise with others concerning the care of patients for whom they are responsible. 

 

To be able to impart relevant knowledge to specific patient groups in appropriate terminology for patient comprehension. 

 

POLICY AND SERVICE DEVELOPMENT 

 

Work within clinical guidelines and have a good working knowledge of national and local standards and monitor own and others quality of practice as appropriate. 

 

Assess and manage clinical risk within your own area. 

 

PLANNING AND ORGANISING 

 

Be responsible for the supervision and co-ordination of junior staff, students and assistants on a daily basis as required by the team leader. 

 

Ensure that you own practice and that of staff under your supervision meet the required professional standards of Physiotherapy practice. 

 

Deputise for the Senior in their absence taking responsibility for operational management of the team, allocating and organising the work of junior and assistant staff to meet service daily. 

 

To be responsible for organising and planning own caseload to meet service and patient priorities in an unpredictable environment. 

 

To be able to demonstrate a working knowledge of department routine procedures e.g. ordering equipment. 

 

ANALYSIS AND DATA MANAGEMENT 

 

Analyse & present data collected during audit or research activities. 

 

RESEARCH, DEVELOPMENT AND AUDIT 

 

Undertake evidence-based projects and develop improvements to service delivery and clinical practice, making recommendation for change to the physiotherapy manager.   

 

Undertake the measurement and evaluation of your work and current practices using evidence-based practice projects, audit and outcome measures, either individually or with more senior physiotherapists. 

 

Be an active member of the in-service training programme by attendance at and participation in, in-service training programmes, tutorials, individual training sessions, external courses and peer review. 

 

To undertake as directed the collection of data for use in service audit and research projects.  To manage and undertake research into specific areas of clinical practice and service delivery using a range of research methodologies as part of MDT audit and departmental research initiatives. 

 

Be actively involved in professional clinical groups.
