Job overview
Cancer Diagnostic Facilitator
To assist the Cancer Services Team in the management of patient pathways, ensuring that patients are reviewed diagnosed and treated within the National Cancer Waiting Time targets.
Assist in the processes that provide Quality Assurance of cancer services that the Trust provide. To collect record and validate a wide range of cancer data as required for the national cancer data sets.
The will include monitoring referrals and whether tests have been requested, reported and acted upon. This will include contacting patients, gathering data and providing the necessary information for real time feedback to external and internal teams.
To liaise with a wide range of departments and staff to gather information required for the cancer MDT meetings. To take responsibility for ensuring this information is available within a set timeframe allowing for informed, timely clinical decisions on treatment plans for patients with cancer.

To contact GP practices and patients to prompt action with regards to at home testing to allow clinical triage to take place in a timely manner. To liaise with various departments to obtain all information required to provide a clinical diagnosis, to prompt for timely diagnostic appointments and reporting.

Detailed job description and main responsibilities
To liaise and support the MDT Coordinators and Pathway Navigators in the management of the patient pathways.
To ensure that all deadlines are met to ensure patient pathways are not compromised.
To liaise with various departments to obtain all information required to provide a clinical diagnosis, to prompt for timely diagnostic appointments and reporting.
To contact GP practices and patients to prompt action with regards to at home testing to allow clinical triage to take place in a timely manner.
To use the Trusts Cancer Registry system and Microsoft Office tools i.e. Excel spread sheets to add and manage relevant date pertaining to waiting times, audit and data.
To produce and distribute daily reports that assist in the management of patient pathways.
To ensure that all case notes are located, retrieved, tracked and made available for the MDT meetings and data validation.
To ensure that all data in regards to the National Cancer Waiting Time (CWT) targets, Cancer Outcomes Services Dataset (COSD) and National Cancer Audits (NCA) is correct by validating information found on the Somerset Cancer Register (SCR), the Patient Administration System, CRIS, Unisoft and patient case notes. To input patient level data, support and liaise with clinical staff of all levels to ensure that data is complete and accurate for all National Cancer Audits. To assist the Cancer Services Management team in the annual national and local Cancer Peer Review process that the Trust must adhere to.
To carry out any other reasonable duties as assigned by the Cancer Management team.

Essential criteria
- • GCSE - O Level Grade A-D or equivalent in Maths and English
- • ECDL or equivalent demonstrable skills
- • Experience of working as part of a team and in isolation.
- • Positive attitude towards change and its implementation
- • Experience of data inputting and quality assurance of data.
- • Experience in customer/ patient services.
- • Competent in the use of Microsoft Office software
- • Ability to communicate effectively with patients, colleagues, clinicians and managers both internal and external to the Trust
- • Ability to work on own initiative and as an effective and responsible team member.
- • Understanding of the National and Local Cancer Targets.
- • Ability to demonstrate flexibility in working hours including out of office core working hours
- • Maintain confidentiality at all times
- • Awareness of Health and Safety in the workplace

- • RSA typing or equivalent or demonstrable keyboard skills
- • Working within a multidisciplinary team

You must have appropriate UK professional registration.
