Colorectal Clinical Support Worker

Band 4

Job overview

The post holder will support the Maidstone and Tunbridge Wells Colorectal Cancer Team. They will provide administrative support to cancer nurse specialists, including assisting with holistic needs assessments and treatment summaries, assisting with organising health and well-being events and helping to provide a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs.

Main duties of the job

·         Triage telephone and email enquiries, using a risk assessment framework, initiating an appropriate response according to protocols and individual pathways (referring complex decisions to the Team for assessment and review); some requiring tact or diplomacy, prompt action and communication.

 

·         Contribute to the holistic needs assessment and the development of individuals care plans, including supporting with actions in line with standard operating procedures and protocols.

 

·         Co-ordinate and organise appointments, assessments and investigations as needed, involving use of a bespoke information system, analysing information produced, ‘sense-checking’ reports produced, highlighting issues to resolve (and providing and/or auctioning a solution). 

 

·         Undertake and assist in direct patient care appropriate to band following competency assessment and required training completed

 

·         Demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration or emergency situation, alerting the team or enabling rapid response.

 

·         Provide support to people affected by cancer, often dealing with highly emotional and sensitive issues and have an ability to manage individual situations effectively.

Detailed job description and main responsibilities

  • Communicating effectively (daily and directly, face-to-face and telephone) with a range of individuals on various levels, e.g.: - patients, relatives, carers, other health / social care professionals and community workers.
  • Providing general information and support about cancer and cancer services to enable navigation of health and social care and to make choices that are best for the individual’s cancer and their life.
  • Promoting equity for patients across the Maidstone and Tunbridge Wells footprint.

Person specification

Qualifications

Essential criteria
  • GCSE English Language and Mathematics (Grades A-C) or equivalent qualification
  • Evidence of continued role development
  • NVQ Level 4 or additional training to diploma level or equivalent in health-related subject; or equivalent knowledge and experience in health / social care field
Desirable criteria
  • Willingness to undertake learning and development

Experience

Essential criteria
  • Understanding of organisational policies and procedures Relevant health and social care experience at AfC Band 3 or equivalent
  • Understanding of person centred care
  • Understanding of health and social care environment, including terminology
  • Understanding of administrative procedures and systems, including non-routine / service specific.
  • Experience of multi-professional working
  • Experience in use of data and database management
  • Experience of good communication skills
Desirable criteria
  • Understanding of Macmillan Cancer Support
  • Awareness of improving outcomes – A strategy for Cancer and other national cancer policy
  • Knowledge of relevant cancer treatments, interventions and terminology
  • Experience in co-ordinating a patient workload
  • Experience of coaching / teaching patients and carers
