Job overview
We are looking for a skilled Therapy Assistant to join our Community Rehab Team based in Tenterden.
We are a multi-disciplinary team of OT's, Physios and Therapy Assistants. We work within patients’ homes and rehabilitation groups to ensure effective discharge planning and a seamless rehabilitation pathway.
Please note as this is a community based role you will need to be able to drive and have access to a vehicle.
Main duties of the job
You will be an integral part o our rehabilitation team, working with our service users, helping them on the road to recovery and keeping them out of hospital so they can live as independently as possible.
Your duties will include:
• Follow up Assessment, planning, implementation and evaluation of delegated caseload and escalating changes to the Registered Practitioner
• To support in the supervision of Band 3 Health Care Support Workers and Trainees
• To actively work as a member of the multi-professional team to provide high quality care to patients/clients
• To undertake, and report back on delegated clinical activities that have previously been signed competent
• To undertake clinical interventions utilising a variety of modes, including planning and leading group and individual therapy and issuing appropriate equipment as part of packages of prescribed care after achieving appropriate competencies.
Additional Working Pattern: 22.5 hours, days to be negotiated

Experience
Essential criteria
- A minimum of 2 years working as a therapy assistant
- Working with patients/clients and families to negotiate and agree a personalised care plan including re-assessing risks and needs
- Be comfortable with liaising with patients and families about a patients plans and discharge plans
Skills and Abilities
Essential criteria
- Be able to communicate with qualified staff, B3 therapy assistants, the entire MDT, patients and relatives
- Effective communication skills both verbal and non-verbal
- Maintaining accurate, legible, comprehensive records for both assessments and review interventions
- Good written note writing and ability to use computerised notes systems
- Working without supervision taking delegated responsibility for the care they give to patients/clients within their own limits of competency and confidence
- Ba able to support, mentor and train band 3 therapy assistants and HCAs
Knowledge
Essential criteria
- Awareness of professional issues and recent developments in the NHS and Integrated Care Board including Clinical Governance, NICE, Essence of Care
- Understands the role and remit of a Band 4

- Know when to escalate concerns to qualified colleagues

You must have appropriate UK professional registration.
