PCN Lead

We are looking for a Band 7 PCN lead to join our Community Nursing teams within the New Romney and Tenterden PCN area. 

You will provide management within the teams of the PCNs. The role will work closely with the operational manager leading on clinical governance and supervision of the community nursing teams.

This post will suit those already working at a senior level with community experience, and also those staff looking for a change to the community setting. This is a fantastic opportunity for those wanting to experience a managerial role, whilst occasionally supporting with clinical work. The Community Nursing Service supports the GP Primary Care Networks, and the successful candidate will require excellent communication skills to build a good relationships, as the go-to person for the GP practises.

The coaching aspect of the role will also support the development of the team’s ability to use their data, to analyse their caseload, local themes and trends, and patient outcomes for professional decision making. This will empower individual responsibilities and enable teams to problem solve and provide support to patients using a holistic approach working together to create individual personalised care planning. Support will be given to those coming for other backgrounds to develop any skills required, and as part of the senior team, staff will be given a buddy colleague for local support.

The role will focus on leadership and coaching and includes overall management to provide assurance of standards, adherence to KPI's, contractual expectations and CQC requirements. You will be responsible for line management of aligned Team Leaders/Band 6 staff, and support proactive management of their caseloads. You will support the senior staff to deliver high standards of care, working closely with, other Specialist Services, GP's, Social Services, Hospice and other MDT partners to provide an integrated approach caring for patients in their own homes. The PCN Lead will focus on enhancing the quality of patient care, and local provision needs, whilst encouraging the development of staff and service.

As the ideal candidate for this role, you will have excellent communication skills, be passionate about empowering patients, and staff, and collaborate with others to deliver high quality care along a care pathway and across organisational boundaries. You will be innovative in how care can be delivered in the future. You will also be prepared to undertake additional education and training to support professional development.

  • Registered health care professional (NMC registration) Health related degree and evidence of relevant post graduate development at Level 6 Evidence of Post Graduate Specialist qualifications relevant to area of practice; specialist practitioner district nursing, non-medical prescribing, mentorship, coaching, and clinical expertise Management or supervisory qualification / programme of study

Experience

  • Minimum of five years’ experience in area of practice Experience of leading and managing teams Experience of developing policy and service development plans with practical experience of change management.
