Physio/OT (Urgent Community Response+ Integrated Therapy Service) - Band 6

£42,939 - £50,697 Inc HCAS per annum pro rata

37.5 hours per week (Monday-Sunday) *7 day roster and the expectation is to work across the Borough of Bromley. 

Full-time & Part time considered - some remote working also. 

An exciting opportunity has arisen for a proactive, dynamic Band 6 Physio/ Occupational Therapist to join our Urgent Community Response Service and the Rapid Access to Therapy service. This role involves ensuring all patients across the borough of Bromley are assigned their therapy interventions in a prompt and efficient manner.

You will work collaboratively with a vast range of key health and social care teams and services, spanning across primary, community and secondary care, as well as voluntary and public sector providers, to ensure the successful delivery of care pathways in Bromley.

There will be opportunities to receive training from senior members of the team, access internal training as part of your CPD and work closely with other urgent community response services. 

Please note that the post holder is expected to work 37.5 hours a week including some weekend working. Part time hours may be considered. 

The band 6s are supported through a monthly in-service program.  Furthermore, you will receive an annual appraisal and regular one to one supervisions with your assigned supervisor.

Main duties of the job

Physio/OT (Urgent Community Response+ Integrated Therapy Service) - Band 6

  • To complete initial Rapid assessments to prevent an immediate hospital admission.
  • Assessment for falling patients in the last 24 hours to 1 week or new significant risk identified that is likely to result in an immediate fall.
  • Urgent Moving and Handling requests to ensure patient can be supported safely at home.
  • Request for urgent walking aid referrals.
  • Post falls assessments to minimise risk of reoccurrence or hospital admission
  • Discharges from A+E- assessments in patients homes. 
  • No family/social support and potential risk identified at home e.g. stairs assessment. · Patient unwell, declining hospital admission, Rapid Response referral, requires urgent M+H/equipment to be made safe overnight.
  • Urgent post discharge issues e.g. discharged without essential equipment/aids, family/carers not managing, significant change in patient functional ability post discharge. ·
  • Palliative/End of life referrals.
  • Therapy goals to work towards with patients.

Experience

Essential criteria
  • Broad range of clinical placements as a student.
  • Band 5 experience
Desirable criteria
  • Previous rehab assistant or band 5/6 experience.

Specific skills

  • • Good communication skills including verbal and non-verbal.
  • • Ability to work in a busy environment
  • • Understands the roles of the wider multidisciplinary team.
  • • Flexible towards other people’s workloads.
  • • Ability to communicate well with families.

Qualifications

Essential criteria
  • • Degree in Physiotherapy/ Occupational Therapy
  • • Registered with HCPC.
  • • Must be a car driver with access to own car for work purposes.
  • • Evidence of CPD.

  • • Computer literate.

Professional Registration

Essential criteria
  • Must have HCPC registration.
Desirable criteria
  • Member of the CSP.

Information Technology

Essential criteria
  • • Has basic IT skills
