Job overview

As a team our aim is to support people living with frailty and their relatives/carers when they are attending the acute hospital, we offer early identification of frailty, comprehensive clinical assessment and person-centred support to individuals, support for carers and education to staff. We are expanding our model of care to include a Frailty Assessment Unit, which brings with it an exciting opportunity to be a part of a passionate team, delivering the highest quality support to people living with Frailty.

Main duties of the job

We are looking for professionals who have previous experience of working in an Acute Frailty area, with evidence of ongoing professional development. You must be highly motivated and possess excellent clinical skills. You will be experienced in working under pressure and have the ability to work independently.

We will provide you with a Trust and Local induction program, leadership and management development, coaching and mentoring opportunities. You will be supported by a dedicated Frailty MDT.
