Liver Surveillance Navigator

We are recruiting for a Cancer Pathway Navigator who shares our ambition and vision to deliver the Best of Care by the Best People and has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.

The Pathway Navigator will work as part of a multi professional team to deliver the Liver Health Checks and Liver Surveillance programme. Key deliverables include work to facilitate a smooth patient journey ensuring that co-ordinated and streamlined administrative processes revolve around patients identified as eligible for cancer surveillance following a screening fibro scan in Liver Health Checks clinics across the network.
The Pathway Navigator will support the patient journey by providing a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs.

Swydd-ddisgrifiad a phrif gyfrifoldebau manwl
The duties and responsibilities listed below are representative of the Pathway Navigator role and its purpose within the service. They are not exhaustive, and the post holder may be required to undertake additional or alternative managerial responsibilities commensurate with the post to support the smooth running of the Hepatology service and the HCC cancer MDT.
Monitor each patient’s pathway by interrogating IT systems such as PAS/OASIS, RIS, iLab in relation to appointments, diagnostics and admissions, and by liaising with Trust colleagues, bearing in mind maximum waiting times for diagnosis and treatment
Inform the Service Manager, Cancer Pathway Manager and Head of Operational Performance of any potential or existing delays in the patient’s pathway that may result in waiting times that exceed appropriate targets
Support the Cancer Referral Office with processing suspected cancer referrals via ERS and Email, validating patient demographics against PAS before registering onto the cancer information system (Infoflex)
Responsible for uploading out-patient appointment details and where necessary, contacting patients to agree appointments dates in keeping with expected national timeframes. Ensure all IT systems reflect important data quality details pertinent to consultant and patient comments regarding upgrade and confirmed appointments. Action copies of referrals in line with Cancer Referral Office processes.
To utilise the cancer information system (Infoflex) in recording of out-patient appointment details, next key events and diagnostic tests for the purpose of the daily Patient Tracker List.

- NVQ 3 in relevant subject or equivalent level of working experience
- Educated to GCSE level including English Language and Maths
Knowledge

- Knowledge of Patient Administration System/OASIS, RIS, iLAB or similar systems
- Knowledge of Cancer Information System (Infoflex)
- Knowledge or awareness of NHS Cancer plan, National Cancer Waiting times and Cancer patient pathways
- Essential understanding of National Cancer Standards
Experience

- Significant experience in an administrative role (cancer experience preferred)
- Experience of communicating with patient/carers via telephone and face to face
- Experience of dealing with deadlines
