Main duties of the job

We need a professionally qualified Social worker/ care manager with a high level of communication skills who can complete complex health and social care assessments in the community for service users with an acute need.

This job is working as part of urgent response care management team to complete timely assessments and provide “wrap-around” packages of care to meet social and health care needs in partnership with therapist and other health staff.

Detailed job description and main responsibilities

  • To work within the integrated team providing a care management service for patients requiring a short-term package of care to prevent hospital admission and facilitate a timely discharge from hospital
  • To provide a 7 day service to support related Health and Social Care Teams working in the acute sector and in the community.
  • To support the Intermediate Care pathway with care management intervention as required and identified by the Operational Manager.
  • To manage a caseload of patients from referral through to discharge or
    transfer of care within a maximum 6-week period.
  • To assess the patient needs, evaluate action required, negotiate for and coordinate the application of resources to provide care.
  • Monitor the outcomes to meet the needs of the patients in order to maintain or adapt to a more normal or safer lifestyle.
  • Work to team, organisation and national objectives to ensure a high quality of patient focused care is maintained
