Job overview

The post holder will be caring and compassionate and have a good nature to work within our frailty wards, be able to work with patients living with dementia and delirium.  Be willing to  provide support by ensuring that our service users are safe whilst promoting their dignity. 

 

Main duties of the job

Undertake a range of delegated clinical care duties to deliver agreed plans of care under the supervision of trained staff

Provide excellent customer care skills to ensure patients come first

To ensure patients receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines

Recognise and avoid situations that may be detrimental to the health and wellbeing of the individuals

To act as an advocate for patients within the department, to ensure a patient orientated approach to the delivery of care

Provides support and assistance to professionals in the implementation of plans of care for individual patients

As directed, to record routine observations of blood pressure, temperature, pulse, respiratory rate and blood glucose when needed. Report any abnormalities and concerns to a registered nurse

Recognise when patients are in pain and discomfort and report to the nurse in charge

Provide reassurance and support to patients, carers and family

Ensure own actions help to maintain quality

To respond to patient requests for non-clinical information to improve communication

Establish and maintain communication with people on routine and operational matters

To act always in a manner which promotes positive working relationships and maintains the professional image of the Trust

Detailed job description and main responsibilities

Under supervision the post holder is responsible for carrying out assigned tasks as part of the clinical team in support of registered nurses, and to undertake some aspects of care provision under supervision. Ensuring the safety and wellbeing of service users, always working in a manner that promotes dignity and human rights, through the adoption of persons centred care principles.

Qualifications

Essential criteria
  • Evidence of a general education e.g. GCSE's or equivalent
  • NVQ II in Healthcare / QCF Diploma in Health Level 2 or equivalent qualification in the area of speciality (e.g. Operating Theatre Practice for posts in theatre) or an equivalent level of working experience within a similar role

Knowledge

Essential criteria
  • Knowledge and experience of taking clinical observations
  • Knowledge of hospital systems

Experience

  • Some experience of working as a healthcare assistant in a hospital or registered care setting
