Tissue Viability Nurse Specialist

An opportunity has arisen for an enthusiastic, highly motivated and experienced nurse to join the Tissue Viability Team within the Dartford & Gravesham NHS Trust.
We are seeking an individual who is experienced in the care of patients with a wide range of wounds/skin conditions. You will be reliable, organized, as well as being kind and compassionate. You will be self-motivated and able to work independently, as well as providing support and leadership to the junior members of the team.
We are looking for someone with excellent communication skills, who will demonstrate a passion and enthusiasm for all things related to tissue viability; teaching and educating staff at all levels, within our organisation.
You will be responsible for the assessment of patients with complex wounds/skin conditions and for the formulation of individualised treatment/management plans, as well as developing new Trust pathways and guidelines.

The post holder will provide highly specialised clinical advice and expertise for patients with multiple and complex wounds and/or skin conditions.
The post holder will maintain effective and efficient systems for pressure ulcer reporting, as well as the interpretation and analysis of RCA findings for the development of Trust-wide action planning and patient pathway development.
The post holder will provide strong leadership that inspires and motivates others to take individual responsibility in pressure ulcer prevention and wound management across the entire patient pathway.
To work as an autonomous practitioner and effectively co-ordinate and deliver a highly specialised service within the field of Tissue Viability.
To develop and deliver tissue viability/ pressure ulcer prevention and management education for all clinical staff across the organisation.
To facilitate audits and research within the field of Tissue Viability.

Main Duties and Responsibilities:
Freedom to act as an autonomous practitioner, managing a caseload of patients with highly complex and multiple needs, act as an effective role model for best practice (evidence-based), and lead in tissue viability, giving highly specialised clinical advice as necessary.
Utilise highly developed specialist knowledge and clinical assessment skills to examine, diagnose, treat and advise on the clinical management for patients with specialist clinical conditions and/or complex wounds. (This includes history taking, physical examination, Doppler assessment and the requesting of diagnostic procedures/clinical investigations, as clinically indicated).
To advise and support other clinical staff in the delivery of quality, holistic and individualised care plans in regard to tissue viability. Lead and assist with complex dressing procedures in the clinical areas and in the operating theatre, as required. Carry out conservative sharp wound debridement, as required.
To carry out assessment and triage of patients with complex vascular, dermatological or surgical problems to ensure patients are put on to the correct pathway and to make direct referrals to the appropriate specialist clinicians/teams.
To identify and participate in new initiatives, which will lead to demonstrable improvements for patients across the organisation, in relation to tissue viability/wounds.
To be responsible for ensuring that all patients/carers and staff have access to relevant up-to-date information regarding the prevention and management of pressure ulcers and of wound care treatments utilised.
To lead on and participate in the development of Trust-wide projects pertinent to tissue viability, e.g. Leg Ulcer Pathways, Pressure Ulcer Pathway. Interpretation and implementation of National Guidelines, ensuring best practice is adopted across the organisation.
To provide specialist advice to associated organisations, where appropriate.
To support senior ward managers and ward sisters and to develop their knowledge and understanding of wounds, pressure ulcer prevention and management. Attend and contribute to the Band 6/7 Sisters’ weekly Huddle, to the discussions on tissue viability and pressure ulcers.
To participate in the investigation process for serious incidents in relation to wounds/pressure ulcers and give highly specialised advice and support to Ward Managers and Matrons in relation to required changes to practice and the formulation of local ward action plans

- BSc/1st degree (health related or equivalent qualification/training and experience).
- Your professional knowledge acquired at degree level will be supplemented by specialist training and experience working within the specialty and short courses equivalent to Masters level.
- Specialist post-graduate qualifications in tissue viability/wound care, leg ulcer management, dermatology and sharp debridement.

- Evidence of teaching skills, including teaching patients in how to manage their own conditions and raise awareness of their condition/s
- Ability to clinically lead and influence staff of all grades/professions.
- Demonstrate up-to-date knowledge of current clinical and professional issues.
- Excellent verbal, written, interpersonal and communication skills.
- Ability to use advanced clinical skills for patient history taking and physical examination to assess, diagnose and deliver direct patient care and/or develop treatment plans.
- Good time management and organisational skills.
- Ability to manage and prioritise multiple and competing tasks, in a constantly changing environment. Ability to analyse and interpret highly complex data and findings
- Competent to carry out conservative sharp wound debridement with good scalpel skills and physical dexterity. Proven clinical photography skills, including the recording and storage of images.
Experience

- Extensive experience of working autonomously at an advanced level within the specialist area, delivering safe and effective patient care.
- Proven evidence of management/leadership skills and autonomous practice.
- Previous experience of student supervision and clinical education at undergraduate/postgraduate level. Experience of delivering patient advocacy.
- Experience of audit, research and evidence based practice. Experience of delivering change management both personally and as a facilitator.
