Job overview

There are eight full time Consultant posts. This post is a replacement post for one of these Consultants who is leaving. We have 16 junior doctors (including 11 CMT) and it is anticipated we will have more CMT trainees from August 2018. We have three SpRs at present

The successful applicant will work alongside (share patient care) another Ageing and Health Consultant (two Consultants per ward) providing two Consultant- led ward rounds per week, reviewing new transfers/ admissions to the ward on a daily basis (internal rota) along with chairing one multidisciplinary team meeting per week.

Main duties of the job

As demand of the Trust’s Stroke Service continues to grow so too have its plans to develop and expand its service and its workforce. As such the Trust is now seeking interest from aspiring physicians looking to further their career ambitions at our hospital.

This is a replacement post and the appointee will be part of the integrated team of Consultant Geriatricians and Consultant Physicians within Dartford & Gravesham NHS Trust. The trust is committed to the development of services for the frail elderly patients.

You will be required to take part in the General Medical on-call rota and will be expected to carry out the required number of PTWRs when on-call.  The average number of admissions in 24 hours is approximately 35.

This postholder will be expected to participate in audit, teaching of undergraduates, postgraduates, nursing and allied health professionals. There is an excellent postgraduate medical centre at Darent Valley Hospital.  This provides lecture and tutorial rooms and a dining room. There are good modern audio-visual facilities and an internal video link connecting the DCU and main theatres with the PGC.   Numerous meetings for hospital doctors and General Practitioners are held at the centre.

Detailed job description and main responsibilities

The Post-Holder will participate in the ‘Geriatrician of the Day’ rota involving reviews of appropriate patients identified by the Frailty specialist nurses in AE short stay wards. This also encompasses seeing Ward referrals and supervising the Urgent Access Clinic. The aim is to manage these patients in the most appropriate setting, which is not always the acute hospital. There will be an increased focus on rapid, comprehensive assessment of selected patients with a view to streamlining pathways, shortening inpatient stays and preventing unnecessary admissions where patients can be more appropriately managed out of hospital.

The appointed candidate will be required to take part in the General Medical on-call rota and will be expected to carry out the required number of PTWRs when on-call. The average number of admissions in 24 hours is approximately 35. On-call duties on the General Medical Rota will currently be approximately one in 16-20. The Trust is developing 7 day services and the organisation of weekend cover may change in the future.

Qualifications

Essential criteria
  • • Full GMC Registration, MRCP
  • • MBBS or equivalent
  • Holds CCT or equivalent or will obtain within 6 months of interview date
  • Applicants that are UK trained, must also be a holder of a Certificate of Completion of Training (CCT)

Clinical Skills

Essential criteria
  • Wide experience in all aspects of Geriatrics and GIM
  • Experience of Multidisciplinary Team working at senior level.
  • Ability to take full and independent responsibility for clinical care of patients at consultant level
Desirable criteria
  • Demonstrates ability in managing common acute and chronic conditions, including emergencies
  • Experience of providing leadership

Management and Leadership Skills:

Essential criteria
  • Experience of service innovation and development Demonstrates an understanding of NHS Management and resources
  • Evidence of effective multidisciplinary team working and leadership supported by multi-source feedback or other workplace-based assessments
  • Evidence of effective leadership in and outside medicine Ability to supervise medical staff
