Colorectal Cancer Support Worker

Trosolwg o'r swydd

The post holder will support cancer patients and their family/friends under the guidance and supervision of a registered practitioner during their cancer journey. This is from referral to diagnosis, through treatment and follow-up with living with and beyond cancer, covering elements of the recovery package which will involve psychological and informational support.

 

Prif ddyletswyddau'r swydd

The post holder will manage and support the self-management interventions workshops which are provided for cancer patients as they complete treatment, and will scope the need and feasibility of other supportive self-management interventions. These may be ‘health and wellbeing’ events, which may require the post holder to work occasional evenings.

They will act as a point of contact for those patients who will have been prepared by their clinical team for self-managing their follow up and will be able to direct patients to appropriate resources and support which will help them to a more effective recovering and rehabilitation.
