We have an exciting opportunity for an experienced nurse or AHP, to work in our friendly community neurological rehabilitation team as the neurological navigator

This role is primarily aimed at improving the experience of people with highly complex long term neurological disabilities, as they journey though the health and community care service system.

The key aim of the role is to provide proactive case management and direct support to services, with expert clinical advice, assessment and education, and directing patient referrals to the most appropriate specialist neurorehabilitation service.

There is a dedicated in service training program, regular clinical and management supervisions, team meetings and a dedicated MDT. There are also opportunities for funded training to further develop clinical or leadership skills.

To support this service, we require an additional Band 7 Nurse/AHP. The position is part time (18.75 hours per week).

Applicants will be considered from any related discipline including Occupational Therapy, Physiotherapy, Speech and Language Therapy, Psychology, Nursing or Social Work.

The successful applicant will have extensive experience working in a range of specialist neurology/neurorehabilitation settings. 

Continuing professional development is actively encouraged and is supported via clinical supervision, annual PDP Plans, post-graduate and in-service training .  All our staff are fully supported by a Clinical Team Lead.

You will be joining a friendly, dynamic and cohesive integrated community therapy team which prides itself on close working relationships with the MDT. Person-centred care and an approach which promotes self-management and independence are at the centre of our philosophy.

We require a person who is an excellent communicator with a flexible approach, who can work autonomously but has close support of colleagues when required.

We strive to use current best practice and do so by regularly reviewing our service and adapting to provide patients with the best possible outcomes using evidence based practice.

HCPC/NMC registration is required for this role. Also a strong team player and proactive problem solver is an advantage. 

It is also essential to hold a full valid driving licence and have access to a car to use regularly for business purposes (unless you have a disability as defined by the Equality Act 2010).

To provide navigation and support to patients diagnosed with a rare or rapidly progressing neurological condition (Motor Neurone Disease, Huntington’s Disease, Cortical Basal Degeneration, Multi-system Atrophy, Progressive Supra-nuclear Palsy).

Working as an active member of the Community neuro team the postholder will provide a specialised initial assessment, provide and co-ordinate information, support services, advice and guidance for these patients from new diagnosis to end of life.

To maintain and develop relationships and pathways with the other key providers for this patient group including palliative care, charitable associations, acute providers, social care and other HCT providers.

Qualifications

  • Degree in nursing or relevant allied health profession eg. Occupational Therapy, Physiotherapy, Speech and Language Therapy
  • Registration with NMC or HCPC
  • Post registration experience in Neurological or palliative care settings
  • Masters degree or equivalent clinical experience
