The post holder will work as a member of the Multi-disciplinary Team providing a high-quality social care service to adults of all ages accessing the support of The Talking Therapies Team.

The social work role in this team is to work alongside therapists to support clients to work through obstacles that may be preventing them from fully engaging with us. This may be at the beginning, during treatment, or toward the end of our services involvement, if intervention would benefit maintaining recovery.  

Obstacles that clients face are of course multifaceted and individualised to the client. The social worker in the team is required to use their skillset to support, outsource/signpost and implement a support plan to assist in removing these barriers. Our focus is  to enable recovery and promote social wellbeing.  

This is a unique role where  social work in its purest form can be exercised. This is to enable, empower and enhance clients’ social circumstances utlising local and statutory services.

Intervention is  implemented using holistic and person centered lens to support the clients individual needs, enhance the service we provide and achieve the desired change to remove obstacles preventing recovery for our service users.

·    Manage care, plan, support, and review care needs in line with policy, procedures, and legislation.

·        Provide direct support and advice to service users and their carers, through information and advice and ensure that those who do not meet Care Act eligibility are appropriately signposted and supported.

·        Ensure appropriate health and social care services are commissioned with and on behalf of service users, including the duty to offer direct payments, and that processes are in place for service users to be charged by the local authority for all social care services.

·        Ensure that commissioned services allow choice to service users and carers and focus on people living in the community with appropriate support and access to universal services.

·   Undertake joint working with Talking Therapy clinicians on complex cases where there is social care needs.

·        Undertake regular risk assessments and ensure that vulnerable people are kept safe.

·        Undertake assessments of carers needs, including identifying contingency plans, and provision of support if appropriate.

·        Liaise and negotiate with other agencies e.g., Health and Community Services, Children’  s Services, Benefit Agency, Housing authorities and providers etc., for joint planning with, or on behalf of, service users.

·        Ensure that accurate computer records of all work undertaken are kept up to date.

·       Develop own knowledge and practice and contribute to the development of policy and evidence-based practice.

·        Continuation of registration with Social Work England to ensure high standards of practice.

·        Participate in regular supervision.

·        Undertake the supervision and/or mentoring of unqualified staff and/or students as required.

·        Contribute to multi-disciplinary working and, as part of that, to be an excellent advocate for social care. 

·       Use effective and appropriate listening and communication skills in order to   gather information, make clinical / practice decisions, create records and refer to clinical reference literature during the communication.

·     Retrieve and use information regarding appropriate health and social care provision, complying with the appropriate regulatory body and within ethical, legislative and organisational frameworks.

  • Significant post qualification experience
  • Knowledge of legislation, theory and policy in relation to relevant care group
  • Experience of undertaking assessments, focussed as personalisation, choice and control, and care planning.
  • Understanding and experience of personal budgets and direct payments
  • Experience of multidisciplinary working and ability to work collaborative.
  • Experience of partnership working with service users and carers, demonstrating dignity and respect
