Job overview

A dynamic new role has been created for a Community Mental Health nurse to work in the FACT service (Flexible Assertive Community Treatment)!

We are seeking to recruit an experienced mental health nurse who is keen to work with service users who require immediate support to prevent escalation and admission to hospital.

You must hold a full valid driving license and have access to a car to use regularly for business purposes (unless you have a disability as defined by the Equality Act 2010).

This role attracts a recruitment incentive of £6,000: £1,500 on commencement, £1,500 after six months and £3,000 every six months thereafter

Main duties of the job

The successful candidate will: 

  • be responsible for the clinical and operational day to day service delivery of care to service users who are receiving support from FACT
  • undertake assessments of needs with recommendations around care planning and risk management
  • undertake Connected Lives assessments including physical health assessments and assessments of medication side-effects
  • participate in Multi Disciplinary Team working
  • supervise Band 5 Nurses, Nursing Associates, Health Care Support Workers and Support Time and Recovery Workers and students.

Detailed job description and main responsibilities

  • To maintain effective joint working with others, developing positive working relationships and clear communications with the FACT team, MDT and other professionals.
  • Ensure that all relevant information regarding service users care in particular issues of concern regarding practice and workload are communicated and documented as necessary to the FACT manager.
  • To liaise with other professionals in relation to service user care, actively participating in care planning and review meetings.

Qualifications

Essential criteria
  • RMN or Dip/BSc (Hons) / MSc in Mental Health Nursing
  • ENB 998/Preceptorship and Mentorship/Practice Supervisor & Assessor training
Desirable criteria
  • ENB 812/BSc in Specialist Community Nursing Practice (Mental Health)
  • Psychosocial Interventions
  • Venepuncture

Experience

Essential criteria
  • Experience of working in a range of settings relevant to the post
  • Post-registration experience with at least one year at Band 5
  • Experience of working in situations of crisis and emotionally demanding environments.
  • Experience and knowledge of safeguarding process
  • Experience of teaching and training
  • Experience and knowledge of working in a multi-ethnic community
  • Experience of delivering evidence-based care
  • Experience of administering medications, assessing side-effects and undertaking physical health checks.
Desirable criteria
  • Experience of working with acute mental health problems in a community setting
